What is the pet-friendly stay agreement form template?
Being pet-friendly brings in guests, but a barking dog left alone in a room on the third floor is a long night for the front desk. This pet stay agreement collects what you need to know about each animal before it arrives — type, size, vaccinations, habits — and asks the one question that prevents most problems: will your pet ever be left alone in the room, and if so, how?
The house rules sit on their own page in plain words: where pets may and may not go, leads in public areas, the pet fee, and what happens if a pet causes damage or distress. Guests choose how they'd like housekeeping to work during the stay, tick a consent and sign with their finger on the signature pad. You can send the link with the booking confirmation so it's done before arrival, or open it at the desk on a tablet.

The pet-friendly stay agreement form, page by page
1 Guest and stay
The guest gives their name, email, mobile, arrival and departure dates, and optionally a booking reference and "Room number (if you know it)".
2 Your pet
The page starts with "How many pets are coming?" (up to three), "Type of pet" (dog, cat or another answer), "Pet name(s)" and breed. Choosing dog asks for a size band. "Are your pet's vaccinations up to date?" is required, and a yes offers an optional upload of the vaccination record. A question asks about flea and tick treatment.
"Anything our team should know?" lists common habits: nervous with strangers, barks or cries when left alone, not used to other animals or children, needs medication, fully house-trained. Ticking any of them opens a box for more detail. "Will your pet ever be left alone in the room?" is required. A yes asks "How will your pet be settled while you're out?" and for a number to call while the guest is away.
3 House rules
A short paragraph sets out the pet rules: pets stay with the guest or settled in the room, no pets in the restaurant or on beds and sofas, dogs on a lead in public areas, a pet fee per stay, and charges for damage, extra cleaning or disturbance. The guest chooses how housekeeping should work, ticks the consent that they're responsible for their pet and will pay the fee and any charges, then signs and dates.

Tips for a better pet-friendly stay agreement form
- Send the agreement with the booking confirmation so problems surface before arrival, not at the desk
- Give housekeeping a daily list of rooms with pets and the guest's housekeeping choice
- Note the guest's out-of-room number on the room's record for the night team
- Keep your pet fee and damage charges visible on your website as well as on the form
- Inspect pet rooms at checkout and note anything while the guest is still there
Every response becomes a PDF in your vault
The signed agreement becomes a PDF in your vault, with the signature drawn in and any vaccination record attached to the same document. You get an email notice, and the guest receives a copy of what they agreed to.
Use the Responses table to see which upcoming arrivals have pets, search by guest or pet name, and export the list to CSV for housekeeping.

Yes. The signature pad works with a finger on phones and tablets, or a mouse on a computer, and guests can type their name instead if they prefer.
Does the form charge the pet fee?
No. It records that the guest agrees to the fee. You add it to the bill through your own system.
Is the vaccination upload required?
Not in the template — it's optional and only offered when the guest says vaccinations are up to date. You can make it required in the designer.
Is this a legal waiver?
No. It's a starting point for recording your house rules and the guest's agreement. Check the wording against your own policies and local rules before you use it.
Can I use it for a holiday rental instead of a hotel?
Yes. Edit the rules paragraph and remove the room number and housekeeping questions if they don't apply.
