What is the corporate volunteer day request form template?
Companies often ask charities to host a team of twenty for a day, sometimes with a fortnight's notice and only a vague idea of what they'd like to do. A good day needs a real task, enough supervisors, the right equipment and, often, a donation to cover the cost of hosting. This corporate volunteer day request form gathers that information up front, so you can say yes to the right requests and plan properly.
The first page asks who the company is and how the day is being organised — paid volunteering days, a team-building day, or staff giving their own time. A gentle question asks whether the company could make a donation towards hosting, with help text explaining why. The second page records team size, a half or full day, preferred and alternative dates, and the type of activity, from outdoor work to skills-based support. Skills-based work asks what the team could offer.
The final page covers practical points: accessibility, under-18s, lunch, transport, permission to share photos, and what success looks like. Each request is filed as a PDF in your FileIt vault.

The corporate volunteer day request form, page by page
1 Your company
The organiser gives the "Company name", their name, job title, email and phone. "How is the day being organised?" offers paid volunteering days, a team-building day, or staff giving their own time. "Could your company make a donation towards hosting the day?" has help text explaining that hosting takes staff time, materials and supervision.
2 Your team and dates
The organiser enters the "Number of volunteers" and chooses "How long?": a morning, an afternoon or a full day. They give a preferred and an alternative date. "What kind of activity interests you?" covers hands-on outdoor work, sorting donations or packing food parcels, skills-based support, helping at an event, a fundraising challenge, or whatever helps most. Choosing skills-based asks "What skills could the team offer?". "Where?" offers your site, their office, or either.
3 Practicalities
The organiser notes accessibility needs or physical limits in the team, and answers "Will anyone under 18 take part?". A yes asks how many and their ages. Lunch can be brought, arranged with the cost passed on, or not needed, and "Getting there" covers cars, minibus or coach, or public transport. They say whether you may share photos of the day and what would make it a success. The booking consent confirms the day is only confirmed by your booking confirmation, and that the team will follow your safety briefing and supervisor.

Tips for a better corporate volunteer day request form
- Only offer tasks that genuinely need doing — volunteers can tell
- Plan one supervisor for every eight to ten volunteers on hands-on days
- Agree the hosting donation before confirming the date
- Send a short briefing with what to wear and where to meet a week before
- Follow up afterwards with photos (if allowed) and the difference the day made
Every response becomes a PDF in your vault
Each request becomes a PDF in your vault and your volunteering team gets an email notice. The company organiser receives a copy of their request.
Use the Responses table to see requests by date and team size, and export to CSV to plan your calendar of team days and track which companies offered a hosting donation.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
