What is the court & pitch booking form template?

Clubs and venues without a full booking system often end up juggling court and pitch requests by phone, text and email. This template collects everything you need in one request: who's booking, which facility, when and for how long, how many players, what it's for, and what they need on the day.

Bookers give their contact details, membership number if they're a member, and any club, team or company. They pick the facility — tennis, padel, squash, badminton, pickleball, basketball, 5-a-side or full-size pitch, sports hall, swimming or running lane, bowling lane or other — whether they want indoor or outdoor, the date, start time, duration and an alternative time. Repeat bookings ask how often and until when.

The second page covers the number of players (with a supervising adult if any are under 18), names, the purpose (with the opposition for league matches), equipment hire, coaching and how they'll pay. The booker agrees to your booking terms before submitting, and each request is saved as a PDF in your FileIt vault.

1 Your booking

Name, email, mobile, whether they're a member (with membership number), and club, team or company. A required facility choice — 'Other' asks which — indoor or outdoor preference, date, start time, duration and an alternative time. A repeat booking asks for weekly, fortnightly or monthly and an end date.

2 Players & extras

The number of players, whether any are under 18 (which asks for a supervising adult), optional player names, and a required purpose from casual games to training, league matches, coaching, events or tournaments. League matches ask for the opposition. Equipment hire covers rackets, balls, bibs, goals or nets, a scoreboard, floodlights and changing rooms. There's a coaching request, accessibility needs, and a required payment choice with a note not to write card details.
