What is the skin care consultation form template?
Before a peel, microneedling or even a deep-cleansing facial, an esthetician needs to know more than skin type. Recent isotretinoin, a retinoid used last night, a history of cold sores or a fresh tan can all change what's safe to do. This consultation form gathers it in one structured record the client fills in before they arrive.
It covers the client's details and the treatment they're booked for, then their skin: type, main concerns, how reactive it is, how it responds to sun and any recent sun exposure. 2 Your skin
A required skin type and checklist of main concerns (acne, scarring, pigmentation, redness, lines, texture, pores, dehydration, sensitivity, congestion), the concern that bothers them most, a 1–5 scale for how easily their skin reacts, how their skin responds to sun, and a required question on tanning, sunburn or sunbed use in the last two weeks.
3 Routine & products
The client's current routine, a required checklist of actives (retinoids, AHA/BHA acids, vitamin C, benzoyl peroxide, scrubs) — any of which asks when they were last used — whether they wear sunscreen daily, treatments in the last 12 months, and past reactions with details.
4 Health check
A short note explains why these questions matter. Required checklists cover medications (including isotretinoin in the last six months, blood thinners, steroids and light-sensitising drugs) and conditions such as pregnancy, diabetes, epilepsy, cold sores, keloid scarring, skin cancer and implants, followed by allergies and an optional doctor's name.
5 Goals & consent
The client describes what they'd like to achieve, any event they're working towards, how long they spend on skin care each day and can upload up to three skin photos. The therapist section records skin analysis, the treatment plan and home care, and a patch test result. The client confirms their information, consents to treatment, optionally agrees to record photos, and signs.

Tips for a better skin care consultation form
- Send the form at booking so you can spot a contraindication before the client travels in
- Re-send it every six months or when a client says their medication has changed

- Ask for photos in daylight, without make-up, from the same angles each time
- Talk through every 'yes' in the health check in person before starting treatment
