What is the salon client consultation form template?
A good salon consultation protects both the client and the stylist. Knowing that someone used box dye in the summer, reacted to a black henna tattoo years ago, or has a sensitive scalp changes what you should do in the chair — and having it written down, signed, is far better than a quick chat over the basin.
This template covers the client's details (with a parent or guardian for under-16s), the services they're booked in for, natural hair type, thickness and condition, and every chemical service in the last 12 months. The health page asks about past reactions, allergies, scalp conditions and medical factors, and records the allergy alert (patch) test result and date. Clients then describe the look they want, upload inspiration photos and a photo of their hair now, and say how much upkeep and budget they have in mind.

1 About you
The client's name, mobile and optional email, whether they're under 16 (which asks for a parent or guardian's name), whether it's their first visit and the appointment date.
2 Your hair
Services booked, from a cut to colour, lightening, perms, keratin, relaxers and extensions. Natural hair type, strand thickness, density and current condition come next, then a required checklist of chemical services in the last 12 months. Ticking henna shows a note about possible reactions with lightener, and any answer other than 'None of these' asks what was done and when. An optional box covers their home routine.
3 Health & sensitivities
A required question on past reactions to hair dye, tattoos (including black henna) or other hair products — a yes asks for details. Clients list allergies and tick any scalp conditions or medical factors such as pregnancy or recent chemotherapy. A required Allergy alert (patch) test question records whether it's been done and the result; any answer except 'Not needed' asks for the test date (or when it's booked), and a reaction shows a clear note not to go ahead with colour.

Tips for a better salon client consultation form
- Send the link with the booking confirmation, so the consultation is done before the client arrives
- Always follow the colour manufacturer's instructions for patch testing — the form records the test, it doesn't replace it
- Ask clients to upload a photo of their hair in daylight; salon lighting can hide colour history
- Update the formula notes every visit so any stylist can repeat the result
- Go through the answers in person as well — the form is a starting point for the conversation
