What is the workplace safety culture survey form template?
Incident rates show what already went wrong. A safety culture (or safety perception) survey asks the people doing the work whether leaders really put safety first, whether they'd report a near miss without fear, and whether controls like PPE and procedures work in practice.
This template covers three areas in agreement grids — leadership and commitment, reporting and learning, and hazards and controls — and asks directly about near misses that went unreported and why. Workers pick the three biggest risks in their area and say how often they see shortcuts.
It's anonymous, with optional site, shift and role only for grouping results. Anyone who wants to report a specific hazard can do it at the end, with its location and a photo, and is reminded to tell a supervisor straight away if it could hurt someone today.

1 About your work
A note confirms no names are collected. Optional questions ask for site, usual shift and role — frontline, supervisor, manager, office or contractor.

Six statements from Strongly disagree to Strongly agree: safety before production or deadlines, supervisors following the rules, leaders talking about safety regularly, money and time to fix problems, proper hazard training, and workers being consulted.
3 Reporting & learning
Five statements on feeling comfortable reporting, not being blamed, problems getting fixed, hearing lessons from incidents, and being able to stop unsafe work. Workers then say whether they reported a hazard or near miss in the last year. Those who held back choose why; those who reported say what happened next.
4 Hazards & controls
Six statements on PPE, equipment maintenance, housekeeping, practical procedures, staffing and workload, and emergency knowledge. Workers pick up to three of the biggest risks in their area and say how often they see shortcuts.
5 Your view
A required 1–10 rating of safety culture overall, one change that would make work safer, and what's working well. A yes/no lets them report a specific hazard now: location, description and up to three photos.

Tips for a better workplace safety culture survey form
- Share it by QR code for workers without email; if several people will use one shared tablet, turn on multiple responses per browser in Settings
- Treat anything reported on the last page as a hazard report and act on it
- Compare frontline and manager answers — gaps are often revealing
- Share results and actions with the workforce to build trust in reporting
- Only show results by group where enough people answered
- Run it on every shift, including nights and weekends
