What is the class reunion registration form template?

Organising a reunion means chasing three things at once: who is coming, what they'll eat, and where everyone else has gone. This class reunion registration form collects all three. Classmates sign up on one link, choose which events they'll attend, and tell you about the people you've lost touch with.

The form is built around a reunion weekend: welcome drinks, a dinner and dance, a family picnic and a tour of the school. Dinner choices only appear for people coming to the dinner, a guest's name and meal only when someone is bringing a guest, and a children count only for the picnic. Even classmates who can't make it are still asked for their news, a favourite memory and old photos — often the best material for the slideshow.

Each sign-up is saved as a PDF in your FileIt vault, photos attached, and the Responses table gives you the guest list, meal counts and T-shirt sizes in one export.

1 About you

Classmates give their name, "Name at school, if different", email, mobile, "Year you graduated or left", form, homeroom or house, and "Where do you live now?". The name at school is useful when people have changed their surname.

2 The reunion

"Will you join us?" offers yes, maybe, or sorry. Anyone not saying no ticks "Which events?" — Friday welcome drinks, Saturday dinner and dance, Sunday family picnic, and a tour of the school. They're asked whether they're bringing a partner or guest, and a yes asks for the guest's name. Choosing the dinner opens "Your dinner choice", and a guest gets their own dinner choice, including not coming to the dinner. The picnic asks how many children are coming.

Attendees can list allergies or dietary needs for themselves and their guest. Everyone can order a reunion T-shirt and tick "Could you help with any of these?" — tracking down classmates, slideshow photos, decorations, setting up, or a short speech.

3 Catch up

The last page is for everyone, coming or not. It has "Your news since school", "Old photos for the slideshow" (up to five files), "A favourite memory from school" and "Classmates we should try to find". Two required questions settle permissions. "May we share your news and photos?" offers with a name, without a name, or for the organisers only. "May we give your email to classmates who ask for it?" is a yes or no.

Make the template yours

  • Remove the T-shirt question, or add a price to the label
  • Set a closing date so sign-ups stop when the caterer needs final numbers
  • Add a co-organiser's email to the notifications
  • Change the accent colour to your school colours

Tips for a better class reunion registration form

  • Share the link in old class group chats and on social media, and ask everyone to pass it on
  • Check "Classmates we should try to find" every week and divide the names among the committee
  • Ask for photos early — slideshows always take longer than you expect
  • Respect the sharing choices: only print news marked to share with a name
  • Use the CSV export for name badges, using the name at school as well as the current name

Every response becomes a PDF in your vault

Each sign-up is filed as a PDF in your vault, with any photos attached, and you receive an email notice. Classmates get a copy of their answers, so they can check their dinner choice later.

Search the Responses table by name or year, and export to CSV to count attendees per event, meals for the caterer and T-shirt sizes. Photos stay with each response, ready for the slideshow.

Do classmates need an account to sign up?

No. They open the link and fill it in, with no sign-up needed.

Can people who can't come still send news?

Yes. The Catch up page is shown to everyone, so classmates who can't make it can still share news, memories and photos.

Does the form take ticket payments?

No. It records who is coming and what they've chosen. Collect payments through your own ticketing or bank details.

What happens to the photos people upload?

They're attached to that person's vault document and listed in their response. The form asks whether they may be shared, and with or without a name.

Can I use it for a one-off dinner instead of a weekend?

Yes. Delete the events you don't need in the designer, and the follow-up questions for them go with them.
