What is the life insurance needs analysis form template?
Recommending the right amount of life cover means knowing what the money would need to do: replace income for a number of years, clear a mortgage and other debts, pay for funerals and education, less whatever savings and existing cover would already provide. This template collects each of those figures from the client before you meet.
It's laid out the way a needs calculation is usually built. The client describes their household and income, chooses how much of it their family would need and for how long, lists debts and future costs, then records savings and any existing personal or workplace cover. Smoking status, a health yes/no and hazardous activities flag what might affect pricing, without asking for full medical detail at this stage.

1 About you
Name, contact details, date of birth, sex (noted as used in pricing), tobacco or nicotine use, and occupation.
2 Family & income
Marital status, number of children — with the youngest child's age asked when there are any — and anyone else who relies on the client's income. Income for the client and partner, then how much of the client's income the family would need and for how many years.
3 Debts & future costs
Mortgage balance and years remaining, other debts, funeral and final expenses, future education costs, and savings or investments that could be used to offset the need.
4 Existing cover & health
Whether any life cover exists; a yes opens death-in-service and personal cover amounts and a box for policy details. A required yes/no asks about significant medical conditions with an optional details box, plus a question on hazardous hobbies or work.
5 What you're looking for

Tips for a better life insurance needs analysis form
- Ask for gross income and let the client choose the percentage — it prompts them to think about what their family would really need
