What is the climbing centre registration form template?
A queue at the front desk on a busy Saturday is usually a queue of people filling in paper registration forms. This template lets climbers register online before their first visit, so staff can see who's new, who needs an induction and who says they can belay, and the signed form is already on file when they walk in.
The first page covers the climber's name, date of birth and contact details, plus an emergency contact. Answering yes to 'Is the climber under 18?' adds the parent or guardian's name and phone, and asks how the young person will climb — with the parent supervising, with another named adult, in a coached session, or unsupervised where your rules allow it for their age.
The experience page records what they'll be doing (bouldering, top-rope, lead, auto-belays, kids' sessions or courses), their experience and usual grade, and 'Can you belay?'. Belayers say which devices they're competent with and whether they can tie in with a figure-of-eight, and can upload a certificate. Everyone chooses an induction. A health checklist, your rules and consents close the form with a signature, and each registration is saved as a PDF in your FileIt vault.

The climbing centre registration form, page by page
1 Climber
Climber's full name, date of birth, a required 'Is the climber under 18?', email, mobile and address. Under-18s add the parent or guardian's name and phone and 'How will the young person climb?' — naming another adult asks for the supervising adult's name. A required emergency contact follows.
2 Experience
'What will you be doing?', climbing experience, and — for anyone who has climbed — the grade they usually climb and where. 'Can you belay?' is required; a yes asks which belay devices they're competent with, whether they can tie in with a figure-of-eight knot and check their partner, and allows an optional certificate upload. Then a required induction choice.
3 Health
A short note, then a required checklist of conditions — heart or blood pressure, epilepsy, diabetes, asthma, recent injury, pregnancy, dizziness — with details asked unless 'None of these' is ticked, and any medication carried.
