1 Renter
The renter gives their full name, email, mobile and date of birth, the type and number of ID shown, where they're staying locally and a photo of the ID.
2 Equipment & rental period
Staff tick what's being rented — city, mountain, road, e-bike, e-scooter, cargo or kids' bikes — the quantity, fleet or frame numbers and rider heights for sizing, then the start and return date and time. Accessories issued include helmets, locks, lights, child seats, panniers, phone mounts and chargers. A required question asks whether every rider will wear a helmet; answering no requires the renter to confirm they were offered helmets and declined. Other riders in the group can be listed.
3 Condition & payment
Staff note the condition at check-out and can upload photos. For e-bikes and e-scooters the battery charge is recorded, with an N/A choice for pedal bikes. The rental charge, deposit held and payment method complete the page.
4 Terms & signature
A short paragraph sets out the renter's responsibilities. The renter accepts the rental terms and a statement about riding responsibly, optionally confirms they accept the terms for any under-18 riders, and signs.

Tips for a better bike & e-scooter rental agreement form
- Photograph each bike at check-out so condition disputes are easy to settle
- Check helmet fit before the renter leaves
- Show e-scooter riders the brakes and speed modes before they set off
- Note fleet numbers so you know exactly which bike went to whom
- Keep the agreement until the deposit is returned and any claims are settled
