What is Modello AA4/8 (codice fiscale)?

The codice fiscale is the 16-character tax code that identifies every person in Italy's dealings with the public administration — and with banks, landlords, employers and doctors. Most people receive one automatically (newborns through their municipality, many foreign nationals through the immigration desk or the Questura when they apply for a permit). Modello AA4/8, from the Agenzia delle Entrate (Italy's revenue agency), is how anyone else asks for one, and how people who already have a code report changes or ask for documents.

One form covers five requests: assigning a new codice fiscale, reporting a change of personal data or address, reporting a death, requesting a codice fiscale certificate, and requesting a duplicate of the codice fiscale card (tesserino) or of the health card (Tessera Sanitaria). It is handed in at any Agenzia delle Entrate office or, for people living abroad, at an Italian embassy or consulate — in person or through someone you delegate.

Who needs to fill out Modello AA4/8 (codice fiscale)?

  • Foreign nationals who need an Italian codice fiscale — for example to study, work, rent a home, open a bank account or buy property — and did not receive one automatically.
  • Italians and foreign nationals living abroad who need a tax code for dealings in Italy; they can apply through the Italian consulate in their country.
  • Parents or guardians asking for a code for a child or a person with limited legal capacity.
  • Heirs who need a tax code for a deceased person, for example for the declaration of succession.
  • People who already have a codice fiscale and need to report changed personal data, a new tax domicile or a foreign residence address.
  • Anyone who needs a codice fiscale certificate, a replacement codice fiscale card, or a duplicate Tessera Sanitaria after it was lost, stolen, never arrived or was damaged.

When to use Modello AA4/8 (codice fiscale)

  • Before you need to give a tax code — for a tenancy, employment contract, university enrolment, bank account or tax return — if you don't yet have one.
  • When your personal details, Italian tax domicile or foreign residence change and the tax register needs updating.
  • When a family member has died and their death must be reported, or an heir needs a code for the deceased.
  • When your health card is lost, stolen, never delivered or damaged. Don't use it for a card that is simply about to expire: a new one is sent automatically to anyone still entitled to health care.

What you need before you start

  • A valid identity document (usually the passport for foreign nationals) — the surname and names on the form must match it exactly.
  • Date and place of birth; for anyone born abroad, the country of birth (the province is then EE).
  • The Italian residence address or tax domicile, split into street type, street name, house number and hamlet — or the full foreign address for people living abroad.
  • The existing codice fiscale, for a change of data, a death, a certificate or a duplicate.
  • The date of death, when reporting a death.
  • Any other tax codes previously assigned to the person, so they can be linked.
  • Your own codice fiscale if you sign as a parent, representative or heir; an organisation's 11-digit tax code if it is the applicant.
  • A list of the documents you attach, and — if someone else will take the form — the delegate's name, place and date of birth and codice fiscale.

How to fill out Modello AA4/8 (codice fiscale), step by step

1 Quadro A, Sezione I — Tipologia richiedente

Tick D if you are asking for yourself (in person or through a delegate), or T if you are asking for someone else — a parent for a child, a legal representative, a taxpayer for a dependent family member claimed on a tax return, an heir for a deceased person, a social-assistance body, a prison, a court, or an organisation such as a bank or pension body that must record the person's tax code.

Only for a new codice fiscale, the two small boxes on the right take the applicant type code from the instructions' table: 01-04 for direct requests (non-resident student, non-resident worker, person temporarily in Italy, other) and 10-18 for requests for a third party (for example 10 for a parent of a minor, 13 for an heir).

2 Quadro A, Sezione II — Tipo richiesta

Tick the request: 1 new codice fiscale, 2 change of data, 3 report a death (with the date of death), 4 codice fiscale certificate, 5 duplicate card or health card. Boxes 2 to 5 need the existing codice fiscale beside them; for a duplicate Tessera Sanitaria, the reason goes in the MOTIVAZIONE box (1 stolen or lost, 2 never delivered, 3 damaged card).

A few combinations are allowed: the certificate (4) together with a new code (1) or a change of data (2), and an heir asking for a deceased person's code ticks 1 and 3 with code 13. Next to box 1 there is also a box to request the codice fiscale card — only for people not entitled to the national health service, because everyone who is receives a Tessera Sanitaria automatically.

3 Quadro B — Dati anagrafici

Required for every request: surname, first names, sex (F or M), municipality or foreign country of birth, province of birth (the car-plate code, such as RM for Rome, or EE if born abroad) and date of birth as day, month, year.

Write names in full exactly as on the identity document, without titles, and without special characters: the instructions give the example Müller, written MUELLER.

4 Quadro C — Residenza anagrafica / domicilio fiscale

Residents of Italy give their registered residence. People living abroad give their Italian tax domicile — where they earn their income in Italy — or else where they stay; if they have neither, they leave it blank.

The address is split into separate boxes: the street type (VIA, PIAZZA, CORSO…), the street name, the house number (with building, staircase or flat) and a hamlet or 'care of' line. The instructions' example: VIA PARIGI 51/B – FRAZ. PRATOLUNGO becomes VIA / PARIGI / 51/B / FRAZ. PRATOLUNGO.

5 Quadro D — Residenza estera

Required, in full and without abbreviations, for anyone who does not live in Italy: country, state or province or county, town, postcode and address.

6 Quadro E — Eventuali altri codici fiscali attribuiti

If the person was given other tax codes in the past — for example a provisional numeric one, or one based on wrong details — list them here so the agency can link them to the code that matches the correct personal details. Leave it blank otherwise.

7 Allegati

List the documents you attach to support the details on the form, such as a copy of the passport, a birth or death certificate, or a statement of why a third party is making the request.

8 Sottoscrizione

Date and sign — the form must be signed by the applicant or it is void. For a request for yourself, that is all. A parent, representative or heir signing for someone else also writes their own codice fiscale in 'codice fiscale sottoscrittore'. When the applicant is an organisation, its 11-digit tax code goes in the first box and the person signing for it gives their own codice fiscale.

9 Delega

Only if someone else will take the form to the office and collect any certificate: the name of the person delegating, the delegate's name, place and date of birth and codice fiscale, the date, and the delegating person's signature. The delegate shows their own valid ID and a copy of the delegating person's.

Common mistakes to avoid

  • Ticking the health-card box with a request for a new codice fiscale when you are entitled to the national health service — the Tessera Sanitaria arrives automatically.
  • Asking for a duplicate Tessera Sanitaria just because it is about to expire; a new card is sent automatically.
  • Writing names with accents, special characters or abbreviations — transliterate them (Müller as MUELLER) and follow the identity document.
  • Putting the whole address in one box instead of splitting street type, street name, house number and hamlet.
  • Forgetting Quadro D when living abroad — every box is required for non-residents.
  • Leaving the applicant type code empty for a new codice fiscale, or filling it in for other requests where it doesn't apply.
  • Not signing, or a delegate turning up without the signed delega and a copy of the delegating person's ID.

Print the completed form, sign it by hand (and sign the delega too if someone else will deliver it), then take it with a valid identity document to any Agenzia delle Entrate office — or, if you live abroad, to the Italian embassy or consulate in your country. How do I get an Italian codice fiscale from abroad?

Can I use this form to replace a lost Tessera Sanitaria?

Yes. Tick box 5, give your codice fiscale and choose the reason: 1 for theft or loss, 2 if it never arrived, 3 for a damaged card. If your card is only about to expire, you don't need to ask.

Which applicant type code should I choose?

Only for a new codice fiscale. For a request for yourself: 01 non-resident student, 02 non-resident worker, 03 person temporarily in Italy, 04 other. For someone else: 10 to 18 — for example 10 parent of a minor, 12 taxpayer with a dependent family member, 13 heir for a deceased person.

Can someone else hand in the form for me?

Yes. Fill in the DELEGA section with the delegate's details and sign it. The delegate brings their own valid identity document and a copy of yours.

My name has an accent. How do I write it?

The instructions ask for no special characters in the name boxes, transliterated into plain Latin letters — the example given is Müller written MUELLER. Italian final accents are commonly written with an apostrophe, as in NICCOLO'.
