What is V-901 — Guardians' consent to a passport for a child under 18?

Form V-901, 'Samþykki vegna útgáfu vegabréfs fyrir einstakling undir 18 ára aldri', is Registers Iceland's (Þjóðskrá Íslands) consent to the issue of a passport for a child under 18. When a passport is applied for a child, both guardians (forsjáraðilar) are expected to attend with the child and show valid ID. If one guardian can't come, that guardian gives written consent on this form, signed in the presence of two witnesses.

The form is in Icelandic and fits on one page: the child's name, kennitala and legal domicile; whether the child has one or two guardians; each guardian's date and place of signing, name, kennitala, phone number, legal domicile, email and signature; two witnesses; and a box for the official who receives it. Registers Iceland checks the registered custody before the passport is issued, and the form warns that giving false information about custody can be punishable under the Passports Act no. 136/1998.

1 Barn — Child

Enter the child's full name (Fullt nafn barns), kennitala and legal domicile (Lögheimili). FileIt checks the kennitala and that the child is under 18, since this consent only applies to minors.

2 Forsjáraðilar — Guardians

Tick whether the child has two guardians (Barnið hefur tvo forsjáraðila) or one (Barnið hefur einn forsjáraðila). By signing, the guardians confirm that the information about custody is correct and that they consent to the passport being issued.

The starred note under the section explains that if it turns out only one parent has custody, both parents' signatures also mean the parent without custody may collect the passport.

3 Guardian details (left and right columns)

Each column has the date and place of signing (Dagsetning og staður), name (Nafn), kennitala, phone number (Símanúmer), legal domicile (Lögheimili), email (Netfang) and signature (Undirskrift). 4 Vitundarvottar að undirskrift — Witnesses to the signature

Only needed when a guardian is not present at the photo session. Two witnesses sign to confirm that the guardian signed the form in their presence; both must be 18 or over, and the form stresses that two witnesses' signatures are always required. Common mistakes to avoid

  • Signing the consent without the two witnesses present — the absent guardian must sign in front of them.
  • Using a witness under 18, or using only one witness.
  • Having a guardian also act as a witness to their own signature.
  • Ticking the wrong number of guardians — Registers Iceland checks the registered custody before issuing the passport, and false information about custody can be punishable.
  • Forgetting a copy of ID for a signer who doesn't have an Icelandic kennitala.
  • Trying to e-sign the form — the consent has to be signed by hand.
