What is UDK 512A — Maternity/parental pay (barselsdagpenge), employees?
UDK 512A is Udbetaling Danmark's Danish-language application for barselsdagpenge — the Danish maternity, paternity and parental benefit — for employees (lønmodtagere). The title on the form limits it to children born or received on 2 August 2022 or later, the date the current Danish leave model took effect.
Normally barselsdagpenge are handled digitally: your employer reports your absence through NemRefusion and you apply on borger.dk. This paper form is for the exceptions the form lists — your employer won't or can't report in NemRefusion, your employer has gone bankrupt, you are exempt from digital self-service, or another reason you explain. It asks about your workplace, the period and reason for your leave, whether you meet the employment requirement, your working hours, your pay, other income and planned holiday.

Who needs to fill out UDK 512A — Maternity/parental pay (barselsdagpenge), employees?
- Employees whose employer won't or can't report their leave in NemRefusion.
- Employees whose employer has gone bankrupt before or during their leave.
- People who are exempt from Denmark's digital self-service and must apply on paper.
- Mothers going on leave four weeks before the due date, or signed off sick because of the pregnancy.
- Fathers and co-mothers taking the two weeks after the birth, or parents taking leave within the child's first year.
- Adoptive parents who are employees, who must also send the adoption documents and a copy of the child's passport.
When to use UDK 512A — Maternity/parental pay (barselsdagpenge), employees
- Apply no later than 8 weeks after the birth or after you receive the child. If you take leave later, apply no later than 8 weeks after your first day of absence; if you had pay during the leave, no later than 8 weeks after your last paid day.
- If you apply after the deadline, the guidance says you can as a rule only get barselsdagpenge from the day Udbetaling Danmark receives the application.
- The deadline can be extended by 7 days if the letter telling you to apply arrived too late, and there are other grounds for dispensation, such as a hospital stay — send documentation explaining why you applied late.
- Use it only when the digital route isn't open to you: the form's own first question is why you are applying on paper.
What you need before you start
- Your CPR number, address and phone number.
- Your workplace's name, address, country and CVR number.
- The first day of the period you are applying for, and the last day if you know it.
- The expected birth date (terminsdato) from your pregnancy record, the scan, or the date of a planned caesarean — or, for an adoption, the child's CPR number.
- Payslips or other proof of your hours in the last 4 full calendar months before the leave.
- Your unemployment insurance fund (a-kasse), if you are a member, with its phone number.
- Your working hours: per weekday, your average week, or your weekend hours — and a copy of your rota if you work shifts.
- Your monthly salary or average hourly wage before the leave, without the labour market contribution (AM-bidrag).
- Details of any other income during the leave and the dates of any paid holiday.

1 Dine oplysninger og arbejdsplads (Your details and workplace)
Your name, CPR number, address and phone number, then your workplace's name, address, country and CVR number in one box. FileIt can take the employer from your vault and prints up to three lines.

Tick one: your employer won't or can't report in NemRefusion, your employer has gone bankrupt, you are exempt from digital self-service, or other — with a short explanation.
3 Periode og årsag til fravær (Period and reason for absence)
Give the first day of the period and, if you know it, the last day. Then tick the reason: mother's leave 4 weeks before the due date, illness connected with the pregnancy, the special nature of the work, publicly laid-down rules, the father's or co-mother's 2 weeks after the birth, leave within the child's first year, or adoption.
For every reason except adoption, give the expected birth date — even if the child has already been born. For adoption, give the child's CPR number and send the adoption organisation's documentation and a copy of the child's passport.
4 Beskæftigelseskrav (Employment requirement)
The key question: were you working on the day your leave started or the day before, and have you worked at least 160 hours in the last 4 full calendar months, with at least 40 hours in each of 3 of those months? If not, give the last date you were employed.
Then say whether you are a member of an a-kasse, which one, and whether you would be entitled to unemployment benefit if you became unemployed. Enclose payslips or other proof of your hours.
5 Ansættelsesforhold og arbejdstid (Employment and working hours)
Say whether you are employed in a flexi job, with a wage subsidy, or neither, and whether you are a temp or casual worker (and if so, whether you worked the day before your leave).
Describe your hours in one of three ways: fixed hours for each weekday, your average weekly hours in the last complete calendar month before the leave, or your Saturday and Sunday hours if you work weekends. Tick the rota box if you work shifts (and send the rota). If you have partly returned to work, give the date and the hours worked week by week.
6 Løn og andre indtægtsforhold (Pay and other income)
Either your fixed monthly salary or your average hourly wage for the last complete calendar month before your first day of absence — both without AM-bidrag, and without extraordinary overtime, holiday pay, ATP or pension contributions.
Then say whether you receive pay or benefits from other work during the absence, where the income comes from, the gross amount per week and your weekly hours in that work.
7 Ferie (Holiday)
If you will take holiday with pay from your employer, give up to three periods. As a rule you can't receive barselsdagpenge while you are paid for holiday, which is why Udbetaling Danmark needs the dates — and why you must tell them if your plans change.
8 Erklæring, samtykke og underskrift (Declaration, consent and signature)
Three yes/no statements: that you spend the leave with your child, that the information is correct, and that you consent to Udbetaling Danmark obtaining and passing on information relevant to the case. FileIt fills in the date; you sign by hand in the Underskrift box.
Common mistakes to avoid
- Using the paper form when your employer can report digitally — the form is for the listed exceptions.
- Missing the 8-week deadline, which can mean barselsdagpenge only from the day the application arrives.
- Writing the actual birth date instead of the due date in the 'Forventet fødselsdato' box. The form asks for the due date even if the child has been born.
- Forgetting payslips or other proof of your hours for the employment requirement.
- Including AM-bidrag, holiday pay, pension or overtime in your pay figure.
- Not mentioning paid holiday during the leave, which can lead to repayment.
- Leaving out the rota for shift work, or the adoption documents and passport copy for an adoption.
- Posting the form without a handwritten signature.

Bankruptcy is one of the reasons listed on the form for applying on paper. Tick that reason and fill in the rest as normal; Udbetaling Danmark decides your entitlement.
