What is UDK 701 — European Health Insurance Card (blue card)?

UDK 701 is Udbetaling Danmark's Danish-language form 'Ansøg om EU-sygesikringskort' — the application for the blue European Health Insurance Card (det blå EU-sygesikringskort, the Danish EHIC). The card entitles you to necessary healthcare on the same terms as the residents of the EU/EEA country you are staying in, and in Switzerland.

Most people get the card for holidays, but the form also covers other situations where special rules can give you a Danish card: studying abroad for up to a year, working or job-seeking abroad, living abroad while working in Denmark as a frontier worker, or living abroad on Danish early retirement pay or unemployment benefit. You can apply for cards for your children under 18 who live at your address on the same form. Udbetaling Danmark asks people to apply digitally on borger.dk where they can; the pink stamp on the PDF says so, and it doesn't print.

Who needs to fill out UDK 701 — European Health Insurance Card (blue card)?

• People living in Denmark and covered by Danish health insurance who are going on holiday in the EU/EEA or Switzerland.
• Students going abroad to study for up to one year.
• People going to work or look for work in another EU/EEA country or Switzerland.
• Frontier workers who live abroad but work in Denmark.
• People who live abroad but receive early retirement pay (efterløn) or unemployment benefit (dagpenge) from Denmark.
• Stateless people, convention refugees and family members of someone insured in Denmark, who may also be entitled to the card.
• Parents applying for cards for their children under 18 living at the same address.

When to use UDK 701 — European Health Insurance Card (blue card)

• Well before you travel: the form's guidance says the card normally arrives at your address within 2-3 weeks.
• If you need proof of cover before the card arrives, the guidance says you can order a temporary certificate (midlertidigt bevis) from Udbetaling Danmark.
• When your card expires. A card for holidays is normally valid for 5 years; a card for other purposes can have a shorter validity.
• When your situation changes — for example you start studying or working abroad — since the card you have may no longer fit your purpose.

What you need before you start

• Your CPR number and phone number.
• Your address split as the form asks: street name, house number and letter, floor, side or door number, postcode and town, and country.
• Which categories describe you: EU/EEA or Swiss citizen, convention refugee, stateless, family member of an insured person, or other.
• What you need the card for — holiday, study, work, job-seeking, frontier work, living abroad on Danish benefits, or another purpose such as a family visit, au pair work or an unpaid internship.
• The names and CPR numbers of any children under 18 living with you (up to seven).
• Anything useful for the remarks box, such as your dates abroad or your school or employer abroad.

How to fill out UDK 701 — European Health Insurance Card (blue card), step by step

1 Dine oplysninger (Your information)

Your name, CPR number, street name, phone number, house number, floor, side or door, postcode and town, and country. The CPR number goes into six boxes, a printed dash, and four boxes; FileIt splits it for you. Fill in the country only if you live outside Denmark.

2 Vælg din situation (Your situation)

Tick every category that fits: citizen of an EU/EEA country or Switzerland, convention refugee, stateless, family member of one of these who is to be covered by Danish social security, or other — with a short explanation. The guidance explains that you can get the card if you live in Denmark, are an EU/EEA or Swiss citizen and are covered by Danish health insurance, and that stateless people, convention refugees and family members may also qualify.

3 Hvad skal du bruge EU-sygesikringskortet til? (What you need the card for)

Tick the first box if you live in Denmark and need the card for holidays. If you live abroad or plan to move abroad, tick one or more of the other purposes: study abroad for at most a year, work abroad, look for work abroad, live abroad but work in Denmark (frontier worker), live abroad but receive efterløn or dagpenge from Denmark, or other.

The form tells you to choose 'other' for things like a family visit, working as an au pair or an unpaid internship, and to say what the purpose is.

4 Kort til familiemedlemmer (Cards for family members)

Say whether you also want cards for your children under 18. They must live at the same address as you. Give each child's name and CPR number — there is room for seven. If you live abroad, the guidance says cards for family members need confirmation from the health insurance authority in your country of residence, and Udbetaling Danmark contacts you if it needs more.

5 Bemærkninger (Remarks)

A free box for anything that helps Udbetaling Danmark decide quickly — for example the dates and place of your studies or work abroad, or why your situation is unusual.

6 Din erklæring, samtykke og underskrift (Declaration, consent and signature)

On page 4 you declare that the information is correct, that you must help clarify the case and report changes that may affect your right to the card, that you know your rights and duties, and that Udbetaling Danmark may get information from employers, unemployment funds, the municipality and other public authorities. FileIt fills in the date; you sign by hand in the Underskrift box. Page 1 and page 2 both remind you: 'Husk at underskrive ansøgningen på side 4' — remember to sign on page 4.

Common mistakes to avoid

• Forgetting to sign on page 4 — the signature box is not on the first page.
• Ticking 'holiday' when you actually live or will live abroad. Tick the purpose that matches your real situation, since special rules apply.
• Listing children who don't live at your address, or who are 18 or older.
• Leaving the 'other' line empty after ticking 'other' in either section.
• Applying at the last minute — the card normally takes 2-3 weeks to arrive.
• Not telling Udbetaling Danmark when you move to or start working in another country.

After you fill it out

Print the form from your FileIt vault, check pages 1 and 2, and sign by hand on page 4. Post it to Udbetaling Danmark, Kongens Vænge 8, 3400 Hillerød — the 'Send til' address on the form. If you can use MitID, Udbetaling Danmark prefers the digital application on borger.dk.

The guidance says the card normally arrives at your address within 2-3 weeks, that Udbetaling Danmark contacts you if it needs more information, and that you get a letter with the reasons if you are not entitled to the card.

Keep the filled PDF in your FileIt vault next to your passport and other travel documents, and note the card's expiry date there too. You must tell Udbetaling Danmark straight away if your situation changes, for example if you move to another country or start work abroad.
