What is IM017 AIR ceasing correspondence and release of information?
The Australian Immunisation Register (AIR) records vaccinations given in Australia. Form IM017, Ceasing correspondence and release of information, lets you tell the AIR that you don't want to receive information from it about your record — or the record of a child under 14 in your care — and/or that you don't want the AIR to share that information with third parties such as vaccination providers. You tick whichever of the four choices apply.
One form covers you and up to three children under 14 in your care; there's room on a separate piece of paper for more. A child on your Medicare card who is 14 or older and wants to keep an existing arrangement completes their own IM017. The form can't be used to update personal details — it only identifies you and establishes your authorisation to update the AIR for this purpose.

1 Reason for completing the form (question 1)
Tick all that apply: you don't want to receive information about your record from the AIR; you don't want the AIR to share your information with third parties; and the same two choices for a child under 14 in your care.
2 Your details (questions 2 to 5)
Your title, family and given names, date of birth, Medicare card number with your reference number, and permanent address.
3 Children under 14 in your care (questions 6 and 7)
Say whether you have children under 14 in your care. If yes, give each child's name, date of birth, Medicare card number and reference number, and home address — up to three on the form.
4 Declaration (question 9)
Declare that the information is complete and correct, then write your full name, sign and date.
Common mistakes to avoid
- Including a child aged 14 or over — they need their own form.
- Ticking a child option without giving the child's details at question 7.
- Giving the wrong reference number — it's the number next to the person's name on the Medicare card.
- Trying to update an address or name on this form — the form says personal details can't be updated with it.
