What is the travel insurance claim form template?

A travel insurance claim usually covers only one or two things that went wrong, but claim forms often ask every question regardless. This travel insurance claim form template starts by asking what you're actually claiming for — cancellation, cutting the trip short, medical treatment abroad, baggage, or delay — and only then asks the specific questions that matter for that kind of claim, so a simple baggage claim doesn't wade through medical questions that don't apply.

Each section is built around what an insurer actually needs to process a claim quickly: a cancellation claim asks why and when it was cancelled, a baggage claim asks what happened to it and for a carrier or police reference, and a medical claim asks for the treatment received and whether the traveller was admitted to hospital. Receipts, booking confirmations and reports can all be attached directly.

Every claim becomes a PDF in the claimant's FileIt vault the moment it's submitted, with every attached document filed alongside it — a complete, organised claim ready to send on, rather than a scattered pile of receipts and screenshots.

The travel insurance claim form, page by page

1 Traveller & policy

The claim starts with the policyholder's name, email, phone and policy or certificate number, plus a space to list any other travellers included in the same claim, with their name and date of birth.

2 Your trip

This page records the destination, departure and planned return dates, the date the trip was booked, and the total trip cost — the basic facts an insurer needs before looking at the specifics of the claim.

3 Your claim

Claimants tick everything they're claiming for: cancelling before departure, cutting the trip short, medical treatment abroad, baggage issues, or travel delay — and the form shows only the matching questions for each. A cancellation claim asks why (illness, bereavement, work reasons, official travel advice, or jury service) and the date it was cancelled with the provider; curtailment asks when the traveller came home early. A medical claim asks for a description of the illness or injury and treatment received, the hospital or clinic and country, and whether the traveller was admitted. A baggage claim asks what happened — lost, delayed, damaged or stolen — the items affected and their value, and any carrier or police reference. A delay claim asks how many hours the delay lasted and the carrier and flight or service number.

Every claim records the total amount being claimed, and whether anything has already been refunded by an airline, hotel or other insurer, with the amount if so.

4 Documents & declaration

Claimants attach booking confirmations and invoices, receipts for the costs being claimed, and — depending on the claim type — medical reports or a doctor's certificate, or a cancellation invoice or evidence of the reason for cancelling. The claim finishes with a declaration that the information given is true and complete, and that the insurer may contact the doctors, carriers and providers named in the claim to verify it, signed to complete the submission.

Make the template yours

  • Add your insurer's specific policy wording or claim reference format to the policy number field's help text
  • Adjust the claim type checklist if your policy covers additional situations, such as travel disruption from a natural event
  • Set the vault folder so claims file alongside other insurance or travel documents
  • Add a question for a preferred payout method if claims are settled directly to the policyholder
  • Increase upload limits if claims for your typical trips involve a large number of receipts
  • Turn on an emailed PDF copy so the claimant keeps their own record of exactly what was submitted

Tips for a better travel insurance claim form

  • Attach every receipt and confirmation you have, even ones that seem minor — insurers often ask for more than expected
  • Get a written report (a property irregularity report, a police report, a doctor's note) as soon as possible after the event, since these are hard to obtain later
  • Be precise about dates and amounts — small inconsistencies are a common reason claims are delayed
  • Declare any amount already refunded by an airline, hotel or other insurer, since insurers cross-check this
  • Keep a copy of the submitted claim yourself, in case the insurer asks follow-up questions later

Every response becomes a PDF in your vault

As soon as a claim is submitted, FileIt saves it as a PDF in your vault, with every receipt, report and confirmation attached, and sends an email notification so nothing sits unseen. If you've turned on a copy for the respondent, the claimant also gets their own PDF record.

From there, most people or agencies handling claims use the Responses table to track claims by type or status, open the individual PDF when following up with the insurer, and export the details to CSV if they're managing several claims across a season of travel.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Travel insurance claim form: frequently asked questions

Is this travel insurance claim form template free?

Yes — it's included with the Forms app on every FileIt account, including the free plan, along with more than 160 other ready-made templates.

Does the claimant need a FileIt account to submit a claim?

No. Anyone with the link can complete and submit the form without creating any account.

Where does the completed claim go?

Every claim is saved automatically as a PDF in the FileIt vault, together with any attached documents, and can be reviewed, filtered or exported from the Responses table.

Does the form only ask about the type of claim I'm making?

Yes — ticking a claim type such as baggage or medical opens only the matching follow-up questions, using conditional logic already built into the template.

Does submitting this form send the claim to my insurer?

No. This form collects and organises everything needed for a claim into one PDF — sending it on to the insurer, through their own portal or process, is a separate step you take afterwards.

Can I claim for more than one thing from the same trip?

Yes — the form lets you tick multiple claim types, such as a delay and a baggage issue together, and shows the relevant questions for each.

Is this form legal or insurance advice?

No. It's a way to organise a claim clearly — whether a specific loss is covered, and for how much, depends on the individual's own policy terms with their insurer.
