1 Traveller

The form opens with Name exactly as in your passport, an optional Preferred name, Date of birth, Gender (noted as sometimes required by airlines and hotels), Email, Mobile — the number the traveller will carry while travelling — and an optional Home address.

2 Passport and travel documents

Nationality, Passport number, Issuing country and Passport expiry date (with a note that many countries require at least six months' validity after return) capture what's needed for bookings, alongside an optional upload of the passport photo page. A Visa question (not needed, already have one, will need one, or not sure) and an optional Frequent flyer number round out the page.

3 Rooming

A required Room type choice — single, twin share or double share — decides what's asked next: anything but single reveals a field for who the traveller would like to share with, and leaving that blank reveals a preference for being paired with women, men or anyone. An optional Seat preference for flights and coaches (window, aisle or no preference) is also collected here.

4 Health, diet and insurance

This page is marked as confidential, shared only with the trip leaders. A checklist covers common dietary requirements and allergies, and a required yes/no on medical conditions, allergies or medication reveals a box for detail, including what to do in an emergency. An optional Mobility or accessibility needs box covers long walks, stairs, heat or altitude, and a required yes/no on travel insurance reveals fields for the insurer, policy number and an optional emergency assistance number.

5 Emergency contact and declaration

The traveller gives an Emergency contact name, relationship, phone and optional email — someone not travelling with the group — before a declaration confirming their details match their passport and agreeing to the organiser sharing them with airlines, hotels and local suppliers only as needed, closed with a signature.
