What is the renters insurance proof form template?

Many leases ask tenants to carry their own renters or contents insurance, often with a minimum amount of liability cover. Collecting proof usually means chasing emails with photos of policy documents that are hard to read and harder to find a year later. A renters insurance proof form asks for the details that matter, in the same order every time, along with the certificate itself.

This template records the policyholder and rental property, then the "Insurance company", "Policy number", cover start date and "Cover ends / renewal date". "What does the policy include?" covers contents, personal liability, accidental damage to the landlord's property, additional living expenses and pet liability; ticking liability asks for the limit, and ticking contents asks for the sum insured. The form also asks whether the landlord is listed on the policy and whether everyone on the lease is covered.

1 Tenant

Policyholder name, email, phone, the rental property and unit, and whether this is a new policy, a renewal or a change of insurer.

2 The policy

Insurer, policy number, cover start and end dates, then a checklist of what the policy includes. Liability cover asks for the liability limit, and contents cover asks for the sum insured. "Is the landlord or management company listed on the policy?" explains what an additional interest is, and asks for the name as listed when the answer is yes. The tenant says whether everyone else on the lease is covered — and who isn't, if not — and can name their agent or broker.

3 Upload & confirm

The certificate or declarations page is uploaded as a PDF or clear photo, up to three files. A final tick confirms the tenant will report cancellations or changes and send proof of renewal before the end date.

Tips for a better renters insurance proof form

  • Ask for proof before keys are handed over
  • Check the dates — an expired certificate isn't proof of current cover
  • Ask to be listed as an additional interest so you hear about cancellations
  • Send a reminder before renewal dates come round
  • Make sure every adult on the lease is covered, not just one
  • Don't advise tenants which policy to buy; just tell them what the lease requires
