What is the tenant pet request form template?
"Can I get a dog?" by text message gives a landlord almost nothing to decide on. This template gives them the full picture: what kind of animal, how many, size and age, whether it's neutered, vaccinated, microchipped and treated for fleas, how it behaves, how long it would be left alone and who cares for it when the tenant is away.
It also covers the questions landlords worry about most — insurance, especially third-party liability, whether the tenant would accept a pet deposit or pet rent, and a reference from a previous landlord if the pet has lived in a rental before. 1 You & the property
Your name, email and phone, the Rental property and unit, the Type of home, and whether there's a private garden or yard.
2 About the pet
Type of pet (choosing Other asks what kind), How many?, the pet's name, Breed, Age, Adult size, Spayed or neutered? and up to three photos.
3 Health, care & behaviour
Vaccinations, with an optional record upload, flea and worm treatment, microchip, a Training and behaviour checklist, the Hours a day the pet would usually be alone, and who cares for it when the tenant is away. If the pet has lived in a rental before, the form asks for the previous landlord's contact details.
4 Insurance & undertaking
Whether the pet is insured — a yes asks about third-party liability cover and the insurer and policy number. The tenant says whether they'd accept a pet deposit or pet rent, adds any other comments, and signs an undertaking covering control, cleaning, damage and waiting for written permission.

Tips for a better tenant pet request form
- Reply in writing whether the answer is yes or no, and list any conditions clearly
- Ask to meet the pet if you can — it often answers questions a form can't
- Check local rules before asking for any extra deposit or pet rent
- Follow up the previous landlord reference rather than treating the contact details as enough
- Note the agreed pet's details in the tenancy file so a later swap for a different animal is a new request
