What is the parking permit application form template?
Parking permits cause more admin than they should: vehicle details written illegibly, missing proof of address, no idea whether someone needs an accessible bay. This template puts each application into the same shape. 1 About you
Name, email and phone, and You are a — resident, staff member, visitor or carer, or contractor. Everyone except staff gives the unit, apartment or house number (for visitors and contractors, the one they're visiting or working at). Residents also give their home address, staff their department and staff ID, those applying for a visitor or carer that person's name, and contractors their company and who they're working for.
2 Your vehicle
Registration / licence plate, make and model, colour, Vehicle type, and Is the vehicle registered to you? (if not, who is). There's an option to add a second vehicle and an upload for registration documents or proof of address.
3 The permit
Permit type — annual, monthly, temporary or visitor — and the Space needed. Choosing an accessible space asks for the accessible parking permit or badge; anything other than an annual permit asks for an end date; staff choose the days they'll park. Applicants can add anything you should consider, then agree to the parking rules.

- Keep a copy of every approved application for disputes about tickets or clamping
