What is the eV charging bay request form template?
As more residents switch to electric cars, building managers and associations are asked to approve chargers in shared garages and car parks. Each request raises the same questions: whose space is it, what charger, how much power, who installs it, how the cable gets there, who pays for the electricity and what happens if the resident moves.
This EV charging bay request template collects the answers. The resident gives their unit, whether they own or rent (tenants attach landlord consent) and whether they want a charger at their own space or access to a shared bay. Vehicle details, charger type, smart load management and how electricity will be paid for come next. Install requests then ask for the installer, their certification and insurance, the cable route, a quote, the cost and whether a survey's been done. The resident acknowledges four conditions on costs, approval, conditions and removal, and signs.
Each request is filed as a PDF in your FileIt vault with the charger spec, quote and insurance attached, ready for the board, landlord or electrical consultant to review.

The eV charging bay request form, page by page
1 Resident & parking space
The resident gives their name, unit, email, phone and whether they own or rent; tenants upload their landlord's written consent. They choose to install a charger at their own space, use a shared bay or either. Requests for their own space ask for the space number and whether it's owned, assigned or leased. Everyone says which car park it is.
2 Vehicle & charger
Vehicle make and model, registration, whether it's fully electric or a plug-in hybrid and typical daily driving come first. Install requests choose a charger type from a standard socket to three-phase, say whether it has smart load management and can upload a product sheet. All requests say how electricity will be paid for, and shared-bay requests describe when they'd use the bay.
3 Installer, costs & approval
Install requests name the installer and their certification, upload insurance, describe the cable route and supply point, attach the quote, enter the cost and say whether a survey's been done. The resident ticks four acknowledgements about paying for the charger and electricity, waiting for written approval, possible conditions and removal, adds any notes and signs.

- Change the charger options to the power ratings your supply can handle
- Remove the shared bay option if you don't have one
- Edit the acknowledgements to match your policy or bylaws
- Add your electrical consultant to the notification list
Tips for a better eV charging bay request form
- Ask for load management on every charger once more than a few are installed
- Check the cable route against fire safety and parking layout before approving
- Keep a register of approved chargers and their circuits from the Responses table
- Agree how electricity will be metered before the first charger goes in
- Check any right-to-charge rules where you are; some limit when a request can be refused
Every response becomes a PDF in your vault
Each signed request is saved as a PDF in the vault folder you choose, with landlord consent, product sheet, insurance and quote attached. You're notified, and the resident can receive a copy.
