What is the insurance policy cancellation form template?

The form asks for the Policyholder's name, email, phone, date of birth and the postcode or ZIP on the policy, which help find the right policy. Are you the policyholder? has a second answer for someone authorised to act for them, which requires an upload of proof of authority such as a power of attorney.
2 The policy
Policy number and Type of policy come first. When should the cover end? offers as soon as possible, a specific date (which asks for the cancellation date) or the next renewal date. The main reason list includes a better price, having sold the car, home or item (which asks for the date of sale), moving abroad, cover through an employer, and Prefer not to say.
Is there a claim open on this policy, or an incident not yet reported? is required, and a Yes asks for the claim reference or brief details.
3 Payments & refunds
The policyholder says whether they've arranged replacement cover, and optionally the new insurer. How do you pay for this policy? matters for the next step. Choosing monthly direct debit shows a note asking them to keep it in place until the cancellation is confirmed. If a refund is due, it can go back to the original card or account, or to a different bank account, which asks for the account details. Optional uploads cover proof of sale or a new policy schedule.
