What is the block party request form template?
Whether you're a council, a homeowners' association or a residents' group, approving a street party means checking the same things every time: which stretch of road, for how long, what's happening on it, how emergency vehicles get through, who's insured and whether the neighbours are on board. This form asks for all of it.
Activities trigger their own follow-ups. A barbecue needs a safety plan, amplified music needs hours, an inflatable needs the hire company's name and selling alcohol needs a licence reference. If any households object, the organiser is asked what the objections were and how they'll be addressed, and they can upload a signed neighbour sign-off sheet.

1 Organiser
The lead organiser gives their name, email, mobile and address, lists any co-organisers and names a residents' group if there is one.
2 Date & location
The organiser names the event, picks a date and a back-up date, start and finish times, the street and the exact stretch of road from and to, and the full closure hours including set-up. They say what kind of road it is, upload a map or sketch, and estimate the number of people and households on the closed stretch.
3 Activities & safety
A checklist covers food, barbecues, amplified music, live bands, children's games, inflatables, sports and stalls. A barbecue asks for a safety plan, amplified music for hours and inflatables for the hire company. Choosing to sell alcohol asks for a licence reference. The organiser lists items on the road, confirms an emergency lane, gives numbers of stewards and first aid cover, says whether there's insurance (with a certificate upload) and describes the clean-up plan.
4 Neighbours & declaration
The organiser says whether every household has been told and how many support or object. Any objections open a question on how they'll be addressed. They can upload a sign-off sheet, explain how vehicle access will be managed if needed, and sign a declaration taking responsibility for the event.

Tips for a better block party request form
- Ask for closure hours that include set-up and clear-up — the party is usually shorter than the closure
- Check the emergency lane answer before anything else; it's the one condition that can't be waived
- Offer organisers a template sign-off sheet so the numbers they give are easy to check
- Follow up on bus routes and through roads early; they usually need extra approval
- Keep approved requests on file — next year's organiser will often reuse the same plan
