What is Form DS-10?
A sworn statement from an older blood relative or the attending physician describing a passport applicant's U.S. birth, used when no birth certificate exists or the birth certificate was filed more than a year after the birth. It goes with the applicant's Form DS-11 and is signed in front of a passport agent, acceptance agent or notary.
