What is Form DS-60?
A sworn statement from someone who has known a passport applicant by both their former and current names, used with Form DS-11 when the name the applicant uses differs substantially from their citizenship evidence and wasn't changed by marriage or court order. It is signed in front of a passport agent, acceptance agent or notary.
