What is UDK 720 — Health insurance country?

UDK 720_ENG is Udbetaling Danmark's English form 'Information about health insurance country'. When you move to Denmark and register with the Danish National Register, your municipality gives you a health insurance card. In some cases another country's social security system covers you, and that country must pay when you use the card — for example if you work in another EU/EEA country or Switzerland, were posted to Denmark by a foreign employer, or get a pension or benefits from another EU/EEA country or Switzerland.

The form's guidelines say you must fill it in if you are over 18 and either a national of an EU/EEA country or Switzerland, or have moved to Denmark from one. It branches: after your nationality and last country of residence, you pick your situation in Denmark — employee or self-employed, job-seeker, student, pensioner, benefit recipient, accompanying family member or other — and answer only that part. Some answers ask you to enclose a European health insurance card, an S1, E 106, E 109, E 121 or S072 form, or an A1 decision.

When to use UDK 720 — Health insurance country

When Udbetaling Danmark or your municipality asks you to provide information about your health insurance country after you move to Denmark.
The guidelines say to submit the form even if information or documentation is missing, and send the missing documentation as soon as you have it.
You must also tell Udbetaling Danmark immediately if your situation changes in a way that may affect where you are covered by social security.

1 A–C. Personal data, nationality and last country of residence

Your personal details and the municipality you moved to. Then your nationality: EU/EEA or Swiss nationals go straight to item D. Others give the country they were last registered in — if it was outside the EU/EEA and Switzerland, go straight to item K and sign.

2 D. Your situation in Denmark

Tick the one that fits after your move: employee or self-employed (E), job-seeker (F), student (G), pensioner (H), recipient of benefits other than pension (I), accompanying family member (straight to K) or other (J).

3 E. Employees and self-employed

Whether you were posted by an employer outside Denmark and, if so, from which EU/EEA country; whether you will work fully or partly in another country, and which. If you state a country, say whether you have a European health insurance card, an E 106, S072 or S1 form, an A1 decision or a refusal of an A1, and enclose a copy.

4 F–I. Job-seekers, students, pensioners and benefit recipients

F: whether you get unemployment benefit from outside Denmark and have a foreign European health insurance card. G: Danish or foreign student, and for foreign students whether you have a card or an E 109, S072 or S1. H: whether you get a pension from abroad, also from Denmark, and whether you have a card or an E 121, S072 or S1. I: which benefits you get from abroad and whether you have a card or an E 106, S072 or S1.

5 J–K. Other, and your declaration

If you ticked 'Other', describe the purpose of your stay. Then date and sign the declaration: the information is correct, you will report changes, and you consent to Udbetaling Danmark getting information from employers, unemployment funds, the local authority and other public authorities.

Common mistakes to avoid

Answering parts that don't apply. Follow the 'go straight to item …' instructions; FileIt shows only your path.
Waiting for documents before sending the form. The guidelines say to send it anyway and follow up with the documents.
Forgetting to sign on page 4.
Sending both a health insurance card and a health insurance form. The guidelines say that if you have both, you only need to send the form.
Assuming a posted worker gets the A1 from Denmark. The guidelines say you apply for an A1 in your country of residence.
