What is SCO power of attorney?

This is the Central Compensation Office's power of attorney form, published by the Swiss Compensation Office (SCO) in Geneva, which handles Swiss old-age and survivors' insurance (OASI, AHV/AVS) and disability insurance (DI, IV/AI) for people living abroad. With it, the insured person authorises a representative to represent them, consult their file, receive all correspondence and act on their behalf with the Central Compensation Office and its units.

You cross at least one box: OASI (account statements, old-age and survivors' benefits and contributions) or DI (including medical data), or both. You can limit how long it lasts. Otherwise the form says it remains valid until revoked. The insured person and the representative both sign, and copies of both identity documents are attached.

People insured under Swiss AHV/AVS or IV/AI who want someone else to deal with the Central Compensation Office for them.
Pensioners abroad who want an adult child or another relative to receive their SCO letters and answer them.
People claiming a Swiss pension or disability benefit through a lawyer, a social worker or another adviser.
Relatives helping an older person who finds letters and forms in a foreign language hard to handle.

Before a representative contacts the SCO about your file, so it can talk to them and send them your letters.
When you start a pension claim or a disability insurance procedure and want help with it.
When a limited power of attorney ends and you want to give a new one, or change representative.

What you need before you start

The insured person's Swiss insurance number (13 digits, starting with 756).
Both people's surnames, first names, dates of birth, addresses (street, postal code, town, country) and e-mail addresses.
A decision on what the power covers: OASI, DI or both.
An end date or event if the power should not last until revoked.
Copies of the insured person's and the representative's identity documents.

How to fill out SCO power of attorney, step by step

1 Swiss insurance number

The form pre-prints '756.' and asks for the other 10 digits. FileIt takes the full 13-digit number, checks its check digit and prints each digit on its own underscore.

2 The insured person

Surname and first name, date of birth, address, postal code, town, country and e-mail of the person whose file it is.

3 The representative

The same details for the person who is given power of attorney.

4 What it covers

Cross at least one box. OASI covers account statements, old-age and survivors' benefits and contributions. DI covers disability insurance, including medical data.

5 Valid until

Optional: a date or an event such as the end of the procedure. The form warns that, unless otherwise stated, the power of attorney remains valid until revoked.

6 Date and signatures

One date, then the signatures of the insured person and of the representative.

Common mistakes to avoid

Crossing neither box. The form asks you to cross at least one.
Crossing DI without meaning to. It gives the representative access to medical data.
Forgetting one of the two signatures, or one of the two identity document copies.
Leaving 'Valid until' blank when you only wanted help for one procedure. Without it, the power lasts until revoked.
Mistyping the insurance number. FileIt checks the check digit, but compare it with the pension letters.

Print the form, have the insured person and the representative sign it, and attach copies of both identity documents.
