What is Form УП-1 (UP-1)?

Form УП-1 (Заявление за отпускане на пенсия(и) и/или добавка(и)) is the application you give the Bulgarian National Social Security Institute (НОИ, NSSI) to be granted a pension, a supplement (добавка), or both. It covers personal pensions (for example for contributory service and age, or disability), survivor's pensions (наследствена), social pensions, and supplements such as the one for care by another person or the one from a deceased spouse.

Page 1 holds the details of the applicant and, where needed, of a legal representative, a consenting parent or guardian, or an authorised person or pension insurance company. Page 2 says what you apply for, how the pension should be paid (at a post office or into a personal account by IBAN) and contains declarations about service in another country, other pensions and, for a survivor's pension, the deceased person. Page 3 chooses how you receive decisions and lists the documents you attach. The last page is the НОИ's instructions.

Who needs to fill out Form УП-1 (UP-1)?

• People reaching retirement who claim a personal pension for contributory service and age.
• People claiming a disability pension or a social pension.
• Family members of a deceased person who claim a survivor's pension, or a widow's or widower's supplement under art. 84 of the Social Insurance Code.
• Pensioners who apply for a supplement, such as the supplement for care by another person.
• Parents or guardians applying for a child under 14, or for a person under full guardianship (as legal representative).
• Authorised persons and pension insurance companies submitting an application with a power of attorney.

When to use Form УП-1 (UP-1)

• When you become entitled to a pension and want it granted.
• When a family member dies and you are entitled to a survivor's pension or the deceased-spouse supplement.
• When you already have a pension and apply for a supplement to it.
• When a minor aged 14 to 18 applies in their own name, with a parent's or guardian's consent on the form.

What you need before you start

• The applicant's full names as in the identity document, ЕГН (or ЛНЧ or НОИ service number) and address for correspondence.
• The same details for a legal representative, consenting parent or guardian, or authorised person, if one is involved.
• A power of attorney if an authorised person submits it: without notarisation for a parent, spouse or child (stating the relationship), notarised for anyone else.
• Your IBAN and a document from your bank showing it, if the pension is to be paid into an account.
• For a survivor's pension: the deceased's full names, ЕГН, date of death and the pension they received.
• Documents for service and insurance income before 1 January 2000: employment record book, УП-2, УП-3 or УП-30 certificates, a social insurance book, or a foreign institution's certificate of service.

How to fill out Form УП-1 (UP-1), step by step

1 Part I: who applies

Enter the НОИ territorial division you apply to and the applicant's details. Fill in the legal representative only for a child under 14 or a person under full guardianship; the parent or guardian consent only for a minor aged 14 to 18 or a person under limited guardianship; and the authorised person or pension insurance company only when they submit with a power of attorney.

2 1. What you apply for

Tick a pension and its kind (personal, survivor's or social), a supplement and its kind, or both. On the line next to them, name the kind of pension, such as contributory service and age, disability or old age, or the kind of supplement.

3 2. How the pension is paid

Choose a post office at a given address, or a personal account with no fixed term by IBAN. With an IBAN, attach a document from the bank. The instructions say to leave this point out when you apply only for a supplement, because it is paid the same way as your existing pension.

4 3. Declarations

Mark whether you have social insurance service in another country (and which), and whether you receive another pension (and which, including one from another country). For a survivor's pension or the widow's supplement, give the deceased person's details, and say whether you have remarried and whether you were adopted under full adoption. Point 3.6 lets you declare other spellings of your names, and point 3.7 reminds you to tell the НОИ within one month about any change that affects the pension.

5 Part II: how you receive decisions

Choose one: by post to your address for correspondence, in person or through an authorised person at the НОИ office, or to your profile in the Secure Electronic Delivery System (ССЕВ).

6 Part III: documents attached

List the documents for service and insurance income before 1 January 2000 and any other documents. Bulgarian service and income after 31.12.1999 are taken from the НОИ's own records.

Common mistakes to avoid

• Filling in both a legal representative and an authorised person. Only one of them submits the application.
• Forgetting the parent's or guardian's consent and signature for an applicant aged 14 to 18.
• Choosing payment by IBAN without attaching the bank's document showing the IBAN.
• Applying for a survivor's pension without the details of the deceased in points 3.3 to 3.5.
• Leaving out service or a pension from another country in points 3.1 and 3.2.
• Bringing a power of attorney without the notarised signature when the authorised person is not a parent, spouse or child.

After you fill it out

Check the PDF, then sign and date it: the applicant signs on pages 2 and 3, and the person submitting it signs the line for the submitter. A parent or guardian giving consent signs on page 1.

Hand it in to the НОИ territorial division with the documents listed in Part III. FileIt doesn't send it for you.

A copy stays in the person's FileIt vault. After the pension is granted, original service documents that aren't kept in the pension file are returned to you.
