This worship team and choir audition form gives everyone who wants to serve through music the same simple way to say so, and gives the worship leader the information needed to plan an audition or a chat.
Applicants choose which team they're interested in — choir, vocals, keys, guitars, bass, drums, strings or wind, the sound desk, or slides and livestream. Choosing the choir asks for a voice part, and strings or wind ask which instrument. Experience levels run from "keen to learn" to trained or professional, with questions about reading music and chord charts and an optional link to a recording. Under-18s are asked for a parent or guardian, who gives consent at the end.
The last page asks which rehearsals and services they could usually make and how often they could serve. Each application is filed as a PDF in your FileIt vault.

1 About you
Applicants give their name, email and mobile and choose an "Age group". Under 16 or 16–17 asks for a parent or guardian and their mobile. "How long have you been part of our community?" runs from less than six months to more than two years, or "I'm not a regular yet".
2 Your music
"Which team are you interested in?" lists choir, lead and backing vocals, keys or piano, acoustic and electric guitar, bass, drums or percussion, strings or wind, the sound desk, and slides and livestream. Choir asks for a "Voice part": soprano, alto, tenor, bass or not sure. Strings or wind asks "Which instrument?".
"Experience" ranges from beginner to trained or professional. Two yes/no questions ask "Do you read music?" and "Can you play or sing from a chord chart?". Applicants can describe their musical background, add a "Link to a recording or video (optional)", and say whether they have their own instrument and equipment.
3 Availability and commitment
"Which could you usually make?" covers a midweek rehearsal, Sunday morning, Sunday evening, and festivals and special services. "How often could you serve?" runs from every week to occasionally. Applicants can say why they'd like to join and choose the best time for an audition. They agree to the team commitment: come to rehearsals, give notice when they can't serve, and follow the team's guidelines. For under-18s, a parent or guardian consent appears as well.

Each application is filed as a PDF in your vault and the worship leader is notified by email. The applicant receives a copy of their answers.
The Responses table lets you search by name or instrument, and the CSV export gives you a sheet of applicants with their parts, experience and availability for planning auditions and rotas.

Yes. Remove the band and tech options in the designer, and keep the voice part question.
