What is the talent show sign-up form template?
Running a talent show means working out a running order from a pile of half-finished slips: an act with no name, a dance group of 'about eight', a song nobody has the track for. This one-page sign-up asks performers, or their parents, for what the stage manager actually needs, so you can build the program and the tech list from the responses.
The form asks for a main contact, the 'Act name' and 'Type of act', every performer's name, how many there are, the length of the act and a short description. 'Music' records whether the act will send a backing track, play live or need no music, along with the song title and artist. Performers tick what they need on stage, such as microphones, a keyboard or a chair, mention any props that take time to set up, and pick the rehearsal times they can make.
'Are any performers under 18?' adds a parent or guardian section: name, a phone number for the night, who will collect the young performers, and a permission statement. The form closes with a photos-and-filming consent and an agreement to your show rules. Every sign-up is filed as a PDF in your FileIt vault.

The talent show sign-up form, page by page
1 Your act
Required main contact, email and mobile, 'Act name', 'Type of act', 'Performer names', number of performers, 'Length of act' and a short description. 'Music' is required. A backing track asks for a link to it, and both tracks and live music ask for the song title. Stage needs, props and required rehearsal times follow. 'Are any performers under 18?' adds the parent or guardian's name and phone, who will collect the performers and a permission consent. Optional photos-and-filming consent, a placeholder for your rules and a required show-rules consent.
Make the template yours
- Change the length options to your time limit
- Add your rehearsal dates in place of the general times
- Ask for a backing track deadline in the help text of the link question
- Add an audition slot question if acts must audition first
- Replace the rules placeholder with your content and judging guidelines
- Set a response limit equal to the number of acts the show can fit
Tips for a better talent show sign-up form
- Sort responses by 'What do you need on stage?' to group acts that share a set-up
- Put a quiet act between two loud ones in the running order
- Collect all backing tracks a week early and test them on the venue's system
- Print the guardian phone numbers for whoever runs backstage
- Close sign-ups a few days before the first rehearsal
Every response becomes a PDF in your vault
Each sign-up is saved as a PDF in your vault, and organisers are emailed. The main contact gets a copy too.
Export the responses to CSV to build the running order, the tech cue list and the program.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
