What is the school club sign-up form template?

A school club sign-up form lets students choose which after-school or lunchtime clubs they'd like to join, ranked in order of preference, while parents confirm how their child will get home and give the consent the school needs. It replaces a paper slip that can get lost in a bag, or a first-come, first-served scramble at the start of term.

Popular clubs usually have limited places, and allocating them fairly is much easier when every student's preferences are recorded consistently from the start. The form starts simply, with Student's name, Class / year / grade and an optional Date of birth — just enough to identify the student without repeating information the school already holds elsewhere.

2 Club choices

A short note explains that places in popular clubs are limited, and that ranking up to three choices helps the school try to give everyone their first choice while having a fallback if a club is full. First choice is required, with optional Second choice and Third choice dropdowns covering clubs like art, chess, choir, coding and robotics, drama, football, netball or basketball, science, gardening, debating, language club and dance. 3 Getting home & consent

How will the student get home after club? covers being collected, walking or cycling home alone, taking a school or public bus, or going on to after-school care, with a Who will usually collect them? field appearing only when collection is chosen. Any medical, dietary or support needs club leaders should know about? opens a required details field whenever the answer is yes, covering allergies, medication and emergency guidance. Parent or guardian's name, Email and Mobile phone complete the contact details, and an optional photo-consent question sits alongside a final consent statement covering participation, possible fees or equipment, and the responsibility to update the school if details change.
