What is the special event permit application form template?

When a town clerk's office, parks department or events team reviews a public event, the questions don't change much from one event to the next: who's running it and how to reach them on the day, where and when, how many people, which roads, how loud, who's keeping it safe and whether there's a bar. This special event permit application asks all of them in a fixed order, so every department reading it finds its part in the same place.

Logic keeps the form proportionate. A small market in a park that doesn't close a road, play amplified music or serve alcohol skips those sections. A road closure asks for the blocks and times, with an optional detour plan. Amplified sound asks for hours and sources. Saying alcohol will be served opens five questions: who holds the license, its number, a copy of it, who will serve and their training, and where it's served and how ages are checked.

Each application is signed and filed as a PDF in your FileIt vault, with the site map, traffic plan, license and insurance certificate attached to the same document.

The special event permit application form, page by page

1 Organiser & event

The organiser gives their name, organisation, "Type of organiser", email, phone and mailing address, plus an "On-site contact on the day". The event gets a name and a type — festival, parade, run, concert, market, rally, filming or something else — and a description of what will happen. "Is this the first time the event has been held?" asks a returning event about anything from past years the town should know.

2 Date, place & crowd

Dates and four times cover the whole footprint: "Set-up begins", "Open to the public from", "Until" and "Site cleared by". The location and "What kind of place is it?" come next, then a required "Site map" upload marking stages, tents, toilets, first aid and exits. The organiser estimates attendance at the busiest time and in total. "Do you need any roads closed?" opens the roads, blocks and closure times and a traffic or detour plan. A checklist covers tents, stages, barricades, generators, inflatables and portable toilets, and a box covers parking and transport.

3 Sound, safety & alcohol

Amplified sound asks for hours and sources. "Who will provide security?" and a required "Security and crowd management plan" cover staffing, lost children, medical emergencies, weather and evacuation, followed by medical cover. Food asks for the number of vendors. "Will alcohol be served?" opens the license holder, number and copy, "Who will serve, and what server training do they have?" and how ages will be checked. The page ends with the trash and clean-up plan, liability insurance with a certificate upload, a declaration and a signature.

Make the template yours

  • Put your notice period, fee schedule and contact office in the cover text
  • Add department addresses — police, fire, public works — to the notification list
  • Change the place types to your own parks and plazas
  • Add questions your code requires, such as a fire inspection for cooking tents
  • Turn the insurance question into a required upload if your town always requires a certificate

Tips for a better special event permit application form

  • Read the site map first — it answers most of the questions the other departments will ask
  • Compare peak attendance with the security staffing before anything else
  • Send the alcohol section straight to whoever handles licensing
  • Use the Responses table to see every event on the same weekend before approving road closures
  • Keep last year's PDF to hand when a returning event applies again

Every response becomes a PDF in your vault

Each signed application is filed as a PDF in the vault folder you choose, with every upload attached. You and any departments on the notification list get an email, and the organiser gets a copy if you leave that on.

The Responses table lists applications with search by event, organiser or location. Export to CSV for a calendar of upcoming events and road closures.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
