What is the food truck event application form template?

Planning food for an event is half menu and half logistics. Organisers need to know what each truck serves, but they also need to know whether it fits the pitch, which side the hatch opens on, whether it needs a hookup or brings a generator, and whether its permit and insurance will still be valid on the day. This template collects all of it in two pages.

The first page covers the truck and the food: business and truck name, contact person, mobile for the day, type of unit, total length and width with the hatch open, which side it serves from, cuisine, a menu with prices and an optional menu photo. Operators tick dietary options and estimate how many customers they can serve per hour.

The second page asks about utilities and safety. 'Power' has three answers. A hookup asks for amps and voltage, and 'Uses a generator' opens questions about generator type, noise level, fuel, whether spare fuel is stored on site (and how), plus a generator safety statement to tick. Water, propane with an optional gas certificate, fire extinguisher, health permit and insurance uploads with expiry dates, a waste plan, your terms and a signature complete it. Each application is saved as a PDF in your FileIt vault.

The food truck event application form, page by page

1 Truck & menu

Required business name, contact person, email and 'Mobile on the day', plus truck name and social links. A required 'Type of unit', 'Total length including hitch (ft)' and 'Width with serving hatch open (ft)', and which side the hatch is on. Required cuisine and 'Menu with prices', an optional menu file, dietary options and customers served per hour.

2 Utilities, safety & documents

Required event dates and 'Power'. A hookup asks for amps and voltage. 'Uses a generator' adds generator type, noise level at 7 m, fuel, 'Will you store spare fuel on site?' (asking how on a yes) and a required safety statement. Then water, propane (with a gas certificate upload), fire extinguisher, required health permit and insurance uploads with expiry dates, a waste plan, a terms placeholder and consent, and a signature.

Make the template yours

  • Change the units to meters if that's how your site is measured
  • Add your available hookup ratings to the help text of the amps question
  • Make the generator option unavailable if your event is hookup-only
  • Replace the terms placeholder with your pitch fee or revenue share
  • Add a question about the time needed to set up and pack down
  • Set a close date well before the event so you have time to plan the site

Tips for a better food truck event application form

  • Check every truck's length against your pitch plan before accepting it
  • Keep generators away from stages and seating, and the noise answer helps you place them
  • Confirm permit and insurance expiry dates fall after the event
  • Sort responses by cuisine to avoid three taco trucks side by side
  • Ask your fire marshal or local authority which safety rules apply to mobile kitchens

Every response becomes a PDF in your vault

Each application is filed as a signed PDF in your vault, with the menu, permits and insurance attached. You're emailed for each new application, and the operator gets a copy.

Export the responses to CSV to plan pitches by size and power, and open a single response to download a truck's documents.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
