What is the soil sample submission form template?

Soil results are only useful when they can be matched to the right field and interpreted for the crop that's going in next. A bag marked "bottom field" with no depth, no crop history and no list of tests leaves the lab guessing and the recommendation generic.

This soil sample submission template is for labs, agronomists and farm advisers collecting samples from growers, and for farms sending their own. The first page records the contact, farm, lab account number, email for results, an optional adviser email and how results should be sent. The second lists every sample with its ID and field, the date sampled, land use, depth, cores per sample and sampling pattern, with an upload for a field map or GPS points.

The third page gives the context for interpreting results: the last two crops, the planned crop and target yield, lime, manure and fertiliser history, the tests requested and any known problems. Each submission is filed as a PDF in your FileIt vault.

The soil sample submission form, page by page

1 Contact & results

Your name, Farm or business, Lab account number, Email for results, Phone and Postal address come first. Agronomist or adviser email lets the lab include them when results go out, and Results by chooses email, post or both.

2 Samples

A note asks for every bag to be labelled with the same ID listed on the form. Date sampled and Number of samples come next, then Sample IDs and fields — one line per sample with the field name and area. Land use, Sampling depth (choosing different depths asks which), Cores per sample and Sampling pattern describe how samples were taken, and a field map, photo or CSV of GPS points can be uploaded.

3 Crop history & tests

Last crop, Crop before that, Crop you plan to grow and Target yield give context. Lime and manure in the last three years each ask for detail, and Fertiliser used last season is optional. Tests requested covers the standard nutrient panel, lime requirement, organic matter, nitrogen, trace elements, texture, CEC, salinity, heavy metals and soil biology. Known problems in these fields and Notes for the lab finish the submission.

Make the template yours

  • Replace the test checklist with your lab's own packages and codes
  • Add prices to test labels so growers know the cost
  • Change depth options to the units your customers use
  • Add a heading or paragraph with your lab's postal address and opening hours
  • Route submissions to the lab's reception inbox
  • File submissions into a vault folder per season or per client

Tips for a better soil sample submission form

  • Sample at the same time of year and depth each time so results can be compared
  • Avoid gateways, troughs and manure heaps when taking cores
  • Take enough cores per sample to represent the whole field
  • Keep sample IDs short and simple to write on a bag
  • Upload a field map so the adviser can see where samples came from

Every response becomes a PDF in your vault

Each submission is saved as a PDF in the vault folder you choose, with any maps attached. The lab is notified straight away, and the grower is emailed a copy to print and put in with the samples.

The Responses table lets you search by grower, field or test and export to CSV to match submissions with incoming samples.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
