What is the landscaping visit report form template?

This template is built for a crew lead on a phone. Services are a checklist, treatment products are required whenever a weed or pest treatment was done, and irrigation questions appear only when the system was checked. Issues and extra-work recommendations are captured so they turn into quotes, not forgotten notes.

1 Visit
The crew lead, crew size, visit date and arrival and departure times are recorded, followed by the client or property, the client's email for their copy, and the site address. The type of visit is scheduled maintenance, one-off job, seasonal clean-up, or callback, and the weather can be noted.
2 Services & materials
A required checklist of fourteen services covers mowing, edging, trimming, hedges, leaves, weeding, pruning, planting, mulching, fertilising, weed or pest treatment, irrigation, litter and blowing, with an "other" option.
Ticking weed or pest treatment requires the products used, rate and area. Ticking the irrigation check requires its status, and a fault requires a description. Areas worked, materials used, how green waste was handled and how much was removed complete the page.
3 Issues & sign-off
Required yes/no questions ask whether there's anything the client should know about and whether extra work should be quoted. Issues are ticked from a list — pests, dead plants, drainage, damaged hardscape, irrigation leaks, tree safety, access — with details required. Photos, total crew hours (required) and the next visit date follow. If the client was on site, they can sign, and the crew lead always signs.
