1 Visit

The report opens with Technician name, email and optional Licence / certification number, the Client and their phone, the Premises address and type, the Visit type (initial survey, treatment, follow-up, routine contract visit, or emergency call-out), Date and Arrival time — the basic record of who, where and when.

2 Findings

A checklist covers Pests found — rats, mice, cockroaches, bed bugs, ants, wasps or bees, fleas, termites, moths, flies, birds, or none found — with a specific safety note appearing if wasps or bees are selected, reminding the technician that bees are protected in many places and a beekeeper relocation may be preferable to treatment. The technician records the Level of activity, Where activity was found, Conditions contributing to the problem (like moisture, clutter or gaps and entry points), Proofing and hygiene recommendations, and can attach up to 10 photos.

3 Treatment

A yes/no records whether treatment was applied — answering no opens a short field for why not, such as a survey-only visit. If treatment was applied, a required checklist covers Methods used (bait stations, spray, dust, heat treatment, fumigation and more), and a detailed Products used field asks for product name, active ingredient, registration number, amount and exactly where it was applied. Optional fields record the number of bait stations or traps in place and a site plan or station map.

4 Safety advice & next visit

A checklist of Safety advice given covers things like keeping children and pets away from treated areas and not moving bait stations, along with a Safe to re-enter after (hours) field. The technician notes what the client should do before the next visit, whether another visit is needed, signs the report, and can capture the client's signature acknowledging the treatment and safety advice were explained, if the client is present.
