What is the safety observation card form template?
Observation cards are one of the simplest leading indicators a safety programme has. Instead of waiting for an incident, people record what they see every day: a team using the right lifting technique, someone standing in the line of fire, a walkway blocked by pallets. The value is in the conversation that follows and in the patterns that build up over hundreds of cards.
Paper cards get lost in pockets and are hard to analyse. This template keeps the same short format but puts it on a phone. The observer records where and when, the work activity, whether it was a safe behaviour, an at-risk behaviour or an unsafe condition, and one or more categories such as PPE, manual handling or vehicles. The second page records the conversation, the immediate action and any follow-up with an owner and target date.
Observers can submit with their name or anonymously. Each card is filed as a PDF in your FileIt vault, and the Responses table lets the safety team filter by category or type to spot trends.

The safety observation card form, page by page
1 Observation
Date, Time, Site and Area or location come first, then Work activity observed. What did you observe? separates safe behaviour worth recognising from at-risk behaviour and unsafe conditions. Category lets the observer tick at least one of ten areas — PPE, body position and line of fire, manual handling, tools, housekeeping, height, vehicles, procedures and permits, electrical and chemicals — with an Other option. What did you see? asks for the behaviour or condition, not the person, and up to three photos can be added if it's safe to take them.
2 Conversation & action
For anything other than safe behaviour, What could this have led to? records the potential outcome. Did you talk with the person or team? leads either to What was discussed and agreed? or to a reason the conversation didn't happen. Safe observations ask whether the behaviour was recognised. Immediate action and Is further action needed? follow; a yes opens Recommended action, Suggested owner and Target date.
3 About you
How would you like to submit this? lets observers include their name, role or company and email, or submit anonymously. Anonymous cards show a short note reminding people that what they typed is still included, so they can check it doesn't identify them.
Make the template yours
- Replace the Site field with a dropdown of your locations
- Edit the categories to match your own safety programme or critical risks
- Remove the anonymous choice if your programme requires named observers
- Add an observation target, such as two cards per supervisor per week, to the cover text
- Add the safety manager's address to notifications so every card, including high-potential ones, reaches them
- Add a QR code to toolbox talk boards and site entrances
Tips for a better safety observation card form
- Aim for a healthy share of safe-behaviour cards — recognition builds participation
- Lead the conversation with a question about why the job is done that way
- Never use observation cards to discipline the person observed
- Close the loop on further actions and tell named observers what changed
- Review categories monthly to choose the next toolbox talk topic
- Fix unsafe conditions before filling in the card
Every response becomes a PDF in your vault
Each card is saved as a PDF titled with the date, with any photos attached, in the vault folder you choose. The safety team is notified straight away. Anonymous cards carry no name or email.
The Responses table lets you filter by observation type, category or potential outcome and export to CSV for trend charts or a safety committee pack.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Is this safety observation card template free?
Yes. It's included in the FileIt Forms app on every account, including the free plan.
How is this different from a near-miss report?
An observation card records everyday behaviours and conditions, including safe ones. A near-miss report records a specific event that could have caused harm.
Can people submit anonymously?
Yes. Choosing Anonymously hides the name, role and email questions.
Can workers fill it in on a phone?
Yes. The form works in any phone browser and photos can be taken straight from the camera.
Do observers need a FileIt account?
No. Anyone with the link can submit a card.
Does the form assign actions automatically?
No. It records the recommended action, a suggested owner and a target date. Follow-up happens in your own process.
