What is the restroom cleaning log form template?
A restroom cleaning log shows customers, staff and managers that washrooms are checked regularly — and gives you a record when someone complains. This template records each visit: which restroom, when, who, whether it was a routine check, a full or deep clean or a response to a complaint, what was done, what was refilled and what needed fixing.
Cleaners mark eight standard tasks Done, Issue or N/A and tick the supplies they refilled. If something's wrong — a blocked toilet, a leak, a broken dryer, vandalism — they pick the problem, describe it, note who it was reported to and add a photo. Ticking needles or sharps shows a reminder not to handle them by hand.

Which restroom (men's, women's, accessible, all-gender, family or staff), its location, date, time, who did it and the type of visit. Responding to a report asks what was reported.
2 Tasks & supplies
An eight-row grid: toilets and urinals, sinks and mirrors, touch points, floors, bins, baby change, walls and partitions, ventilation and lighting — each Done, Issue or N/A. Supplies refilled is a quick checklist; running low in the store cupboard asks which supplies need ordering. For anything other than a routine check, the form asks whether a wet floor sign was put out.
3 Issues & sign-off
Any problems found? is required. Yes asks what kind — blocked, leaking, broken, dryer, lighting, vandalism, sharps, bodily fluids — with details, whether the restroom was closed, who it was reported to and photos. Choosing sharps shows a safety reminder. Initials and a confirmation finish the check.
