What is the property information questionnaire form template?

Every home sale brings the same questions from the buyer's side: whose fence is it, has there been a dispute with the neighbours, was the extension approved, is there a warranty on the windows, has it ever flooded, and what's staying behind. Getting the seller's answers early, in writing and signed, saves weeks of back-and-forth once an offer is accepted.

This property information questionnaire walks a seller through those topics in five pages. It records the seller, address, tenure and who lives there; boundaries, disputes and rights of way; alterations, approvals, guarantees and notices, with uploads for certificates; services, heating, electrics, flooding, insurance and environmental issues; and finally what's included or excluded, whether the sale depends on an onward purchase, and a signed confirmation. Most questions allow "Not known", because a guess is worse than an honest gap.

It's a general questionnaire, not a jurisdiction's official disclosure form. Where your market has a mandatory seller form, use it as well; this template helps the seller gather the answers and documents in one place. Each completed questionnaire is filed as a PDF in your FileIt vault with its attachments.

The property information questionnaire form, page by page

1 Property & seller

The seller gives their name, any other sellers on the title, contact details and the property address, then chooses the tenure, the date they've owned it from and who lives there. If anyone lives there, the form asks whether other adults aged 17 or over occupy it.

2 Boundaries & disputes

The seller says who maintains the boundaries and which is whose, whether any boundary has moved, whether there have been disputes or complaints (with details), whether anything might lead to one, and whether anyone else has a right of way or shared access.

3 Alterations, guarantees & notices

A checklist records extensions, conversions, removed walls, windows, rewiring, heating, roofing and change of use, with dates and who did the work, and whether planning or building approvals were obtained. Approvals and guarantees can be uploaded, and the seller notes any notices or proposals affecting the property.

4 Services, flooding & environment

The seller ticks the services connected, describes the heating and its last service, the last electrical inspection and where the meters and stopcock are. They say whether the property or garden has flooded, with when and why, whether insurance has been hard to get, and whether they know of radon testing, invasive plants, subsidence, asbestos, damp or contamination. Parking arrangements finish the page.

5 Fixtures & sign-off

A checklist shows what's included in the sale, with a box for items being removed or sold separately. The seller says whether the sale depends on an onward purchase and whether the property will be empty on completion, then confirms the answers and signs.

Make the template yours

  • Replace the questions with your jurisdiction's required disclosure topics where they differ
  • Add a leasehold or condominium section asking for the management company, service charges and ground rent
  • Email the seller a personal link from your agency so the response arrives with their name and email
  • Add your conveyancer or lawyer to the notification list
  • Change the confirmation wording after taking local legal advice

Tips for a better property information questionnaire form

  • Ask the seller to upload approvals and guarantees now; finding them later delays exchange
  • Treat "Not known" as a prompt for your own checks, not a red flag
  • Check the alterations list against the listing photos and floor plan
  • Remind sellers that answers must be updated if anything changes before completion
  • Keep the signed PDF on the transaction file

Every response becomes a PDF in your vault

The completed questionnaire is saved as a PDF in the vault folder you choose, with approvals, certificates and guarantees attached. You're notified, and the seller can receive a copy for their records.

From the Responses table you can open any seller's answers, share the PDF with the buyer's side, or export answers to CSV.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
