The music lesson registration form, page by page
1 Student
The student's name and age are required, along with whether they're under 18 — a yes asks for their school. An optional box lets the family share anything the teacher should know, such as learning differences or how the student learns best.
2 Instrument & level
A dropdown covers piano, guitar, bass, violin, viola, cello, flute, clarinet, saxophone, trumpet, drums, voice, ukulele and music theory, with an 'Other' that asks which instrument. Required questions ask whether there's an instrument at home to practise on and the student's level; anyone beyond complete beginner is asked for years playing. Optional fields cover reading music, graded exams passed, goals and favourite styles.
3 Lessons
Families choose studio, home or online lessons — home lessons ask for the address — plus 30, 45 or 60 minutes and weekly, fortnightly or a trial lesson first. Days and times that work are both required checklists, followed by an earliest start date and any preferred teacher.
4 Contact & terms
A parent or guardian's name is required when the student is under 18. Email and phone are required for everyone, with optional questions on how they heard about you and photo or video permission for recitals. A consent statement confirms lessons are booked for the term in advance and missed lessons follow the studio's notice and make-up policy.
