What is the heat stress check form template?

Heat illness builds up quietly. A crew that's fine at 9am can be struggling by 2pm, especially when the work is heavy, the PPE traps heat or some of the team are new to the conditions. Supervisors who check conditions and controls through the day, and write down what they decided, are better placed to act before someone collapses.

This heat stress check template keeps the check short enough to repeat. The first page records the site, task, crew size, air temperature, humidity, an optional heat index or WBGT reading, whether a heat warning is in force, sun exposure, work rate and heavy clothing. The second page is a grid of common controls — water, reminders to drink, shade, rest breaks, rescheduling heavy work, job rotation, buddies, acclimatisation, cooling aids, awareness and first aid — plus the work and rest schedule and water supply.

The last page asks about symptoms, shows an emergency reminder when anyone is affected, and records the action taken, the work decision and the next check. Each signed check is filed as a PDF in your FileIt vault.

The heat stress check form, page by page

1 Conditions

Supervisor, an optional email for a copy, Date, Time, Site, Workers on this task and Task or activity come first. Air temperature with its Unit, Relative humidity and an optional Heat index or WBGT reading describe the weather. Is a heat warning in force today?, Exposure, Work rate and Heavy or impermeable clothing worn? describe the heat load on the crew.

2 Controls

Controls in place is a grid of eleven controls answered yes, no or N/A, from cool drinking water close to the work to a first aid and emergency plan. Work and rest schedule, New or returning workers on this task, Water supply and Controls missing and what will be done complete the page.

3 Worker check & decision

Has anyone reported or shown symptoms of heat illness? opens a symptoms checklist, from heavy sweating and cramps to confusion, hot dry skin and collapse, with a reminder that heat stroke is a medical emergency. The form then asks how many workers were affected and what action was taken, with help text to avoid health details that aren't needed. Work decision records whether work continues, continues with extra controls, is rescheduled or stopped. Next check and the supervisor's signature finish the record.

Make the template yours

  • Add a site dropdown for teams working across several locations
  • Add the safety manager to notification emails so stopped or rescheduled work is seen quickly

Tips for a better heat stress check form

  • Check at the start of the shift and again at the hottest part of the day
  • Pay extra attention to new and returning workers in their first days
  • Put water and shade close to the work, not back at the vehicle
  • Move heavy tasks to early morning when you can
  • Never wait to fill in the form when someone may have heat stroke — get help first
