What is the rental guarantor application form template?
When a tenant needs a guarantor — a student renting for the first time, someone new to the country, or an applicant whose income falls short — the landlord or agent needs the guarantor's own information to decide whether they can support the tenancy. A rental guarantor application form gathers that in one go: who they are, where they live, what they earn, their relationship to the tenant, and their supporting documents.
This template asks for identity and address history, with a previous address appearing when the guarantor has been at their current one for under three years. It records the tenancy being guaranteed and, for shared tenancies, shows a plain-language note that a guarantee may cover the whole rent. Employment questions change for employed and self-employed guarantors. It also asks about other guarantees and any court judgments or insolvency arrangements, then collects photo ID, proof of address and proof of income, consent to checks and a signature.

The rental guarantor application form, page by page
1 About you
Full name, date of birth, email, phone and current address, then whether the guarantor owns, rents or lives with family, and "Years at this address" — under three asks for a previous address.
2 The tenancy
The tenant being guaranteed, "Your relationship to the tenant", the property and monthly rent. If it's a shared tenancy, a note explains the guarantee may cover the whole rent and suggests reading the agreement carefully.
3 Employment & income
Employment status opens the right follow-ups: employer, job title, start date and an HR contact for employed guarantors; business name, years trading and accountant for the self-employed. Gross annual income and other income follow. The form asks whether they already guarantee anyone else, and about court judgments, bankruptcies or insolvency arrangements in the last six years, with an explanation if so.
4 Documents & consent
Uploads for photo ID, proof of address and proof of income, a consent to identity, credit and reference checks, and the guarantor's signature.
