What is the farm biosecurity visitor log form template?

Vets, hauliers, feed drivers, contractors, buyers and school groups all bring something onto a farm, and occasionally that includes disease. A biosecurity visitor log does two jobs: it makes visitors stop and think about where they've been before they walk in, and it gives the farm a record of who came, when and where they went if an outbreak is ever traced.

This template works on a visitor's phone from a QR code at the farm gate. The first page records the visitor, company, mobile number, vehicle registration, date, arrival time, reason for visit and host. The second asks when they last had contact with livestock or another farm, which animals, which farms or markets, any travel abroad and contact with animals there, and whether they've recently been on a farm with a disease outbreak — which shows a note asking them to wait by their vehicle.

The final page records the precautions taken: vehicle, wheel wash, footwear, clothing, hand washing, areas to be visited and any equipment brought in, followed by agreement to the farm's rules and a signature. Each visit is filed as a PDF in your FileIt vault.

The farm biosecurity visitor log form, page by page

1 Visitor

Your name, Company or organisation and Mobile phone come first — the help text explains the farm may need to contact them if a disease is later confirmed. Email, Vehicle registration, Date of visit, Arrival time, Reason for visit and Who are you visiting? follow.

2 Recent contact

When did you last have contact with livestock or another farm? ranges from today to not that I know of. Any contact asks Which animals? and Farms or markets visited in the last 7 days. Have you been abroad in the last 14 days? asks which countries and whether they visited a farm there. A yes to a recent disease outbreak shows a note asking the visitor not to go further and to speak to the farm manager.

3 Precautions & agreement

Your vehicle records whether it was parked in the visitor area, cleaned first or driven on as essential, with a wheel wash question for vehicles driven on. Footwear, Clothing and Hands washed or sanitised on arrival? follow. Areas you will visit lists the office or yard, housing, parlour, calf or lambing areas, feed store, fields, poultry sheds and pig units. Bringing equipment onto the farm? asks what and how it was cleaned. The visitor agrees to the farm's biosecurity rules, gives an expected departure time and signs.

Make the template yours

  • Change the contact periods to match your vet's or scheme's advice
  • Print a QR code for the form link on the farm gate sign
  • Add the farm manager to notification emails so every sign-in, including any outbreak answer, reaches them
  • File visits into a vault folder per year

Tips for a better farm biosecurity visitor log form

  • Put the QR code where visitors stop, before the yard, not at the office door
  • Keep a footbath and hand sanitiser next to the sign
  • Have a clear place for visitors to park off the yard
  • Check the log regularly to see who comes most and what they bring in
  • Export the visitor list to CSV if your vet ever needs to trace contacts

Every response becomes a PDF in your vault

Each visit is saved as a signed PDF in the vault folder you choose, titled with the date and visitor's name. You're notified straight away, and the visitor can receive a copy.

The Responses table lets you search by date, visitor or reason and export to CSV, which is the quickest way to answer "who was on the farm last week?" if a disease is investigated.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
