What is the family reunion registration form template?

Whoever organises a family reunion ends up being the switchboard: who's coming, how old the kids are now, how many shirts in which sizes, who's in the hotel and who's bringing the potato salad. This registration form puts all of that in one place. Each household fills it in once, adding every person in their group, and you get the numbers you need for the caterer, the printer and the hotel.

The first page is the group. The person registering gives their contact details, which branch of the family they belong to, their own age and T-shirt size, and how many people are coming. Say four, and three short guest sections appear, each asking for a name, relationship, age and shirt size. The second page covers the event: which days the group will attend, where they'll stay (with a room count if they're in the reunion hotel block), mobility needs, dietary needs and what they'll bring to the potluck.

There's room for the fun parts too — old photos for the family history table and news to read out on the day. Every registration is filed as a PDF in your FileIt vault.

The family reunion registration form, page by page

1 Your group

The person registering gives their name, email, mobile and an optional home address for mailing the family photo. "Which branch of the family are you from?" helps you group households. They give their age and T-shirt size, then answer "How many people in your group, including you?" with a number up to six. For each extra person a short section appears: Guest 2 name, relationship to you, age and T-shirt size, from youth small to adult 3XL, or "No shirt".

2 Days, lodging & food

"Which days will your group attend?" lists the Friday welcome evening, Saturday picnic and program, and Sunday farewell brunch. "Where will you stay?" offers the reunion hotel block, family nearby, camping or RV, their own booking or day visits only. The hotel block asks how many rooms and which nights. Next come mobility or accessibility needs and a dietary checklist. "What will you bring to the potluck?" opens a question about the dish and how many it serves, unless they're bringing nothing this time. The page ends with volunteer jobs, up to five old family photos, family news to share and permission to include the group in the family album.

Make the template yours

  • Rename the three days to match your schedule, or cut them to one
  • Put the hotel name, booking code and room block deadline in the cover text
  • Add a currency question for a registration or T-shirt fee and explain how to pay
  • Raise the group size limit by duplicating a guest section in the designer
  • Set a close date that matches your T-shirt order deadline

Tips for a better family reunion registration form

  • Export the Responses table and count T-shirt sizes across all guests before ordering, with a few spares
  • Share the hotel room count with the hotel well before the block releases
  • Sort the potluck answers by dish type and gently nudge late registrants toward what's missing
  • Print the family news and old photos for a display — it's often the most visited table
  • Keep this year's responses; next time you'll have everyone's details to start from

Every response becomes a PDF in your vault

Each household's registration is filed as a PDF in the vault folder you pick, with any photos they uploaded attached. You get an email notice, and the person who registered gets a copy if you've left that on.

The Responses table shows every household with search across answers. Export to CSV to total the headcount by day, tally T-shirt sizes and build the potluck list.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
