What is the driving lesson booking form template?
Before a first driving lesson an instructor needs to know who the learner is, what licence they hold, how much driving they've done, whether a test is booked, and where to pick them up. Collecting that over the phone is slow and easy to get wrong.

1 About you
The learner enters their name as it appears on their licence, date of birth, mobile and email.
2 Licence & experience
The learner chooses their licence status: learner permit or provisional licence, a full licence from another country, a full licence wanting refresher lessons, or not yet applied. Anyone with a licence is asked for its number and expiry date. Theory test status, driving experience and private practice follow, and if a practical test is booked the form asks for the date and test centre.
3 Lessons
Required: manual, automatic or no preference, and the kind of lessons — single, a block (which asks for 5, 10 or 20 hours), intensive, refresher, motorway or highway, or a post-test course. Lesson length, available days and times, the earliest first lesson and any instructor preference follow.
4 Pickup & terms
The pickup address is required, with an optional different drop-off address such as school or work. A required question asks about medical conditions or medicine that could affect driving, noting that some conditions must be declared to the licensing authority. The learner confirms their eyesight meets the legal standard and agrees to bring their licence to every lesson and to the school's cancellation terms.
