What is the teen driver agreement form template?

When a teenager gets their licence, most families talk about the rules. Fewer write them down, which makes it harder to remember what was agreed about passengers, curfew or who pays for fuel once the first argument comes along.

This template turns that conversation into a signed agreement over four pages. It records the driver, licence stage, the car and a review date. The rules page starts with a reminder that graduated licensing laws set the minimum, then covers passengers, school-night and weekend curfews, conditions where a parent must be in the car, phone rules and a set of always-rules the teen ticks. The third page splits costs and car care, sets consequences for each kind of broken rule in a grid, lists rewards and includes a no-questions-asked safe ride promise. Teen and parent sign at the end.

The signed agreement is saved as a PDF in your FileIt vault, next to the teen's licence and the car's insurance.

The teen driver agreement form, page by page

1 Driver & car

The teen driver's name and date of birth, licence stage, optional issue date, the vehicle they'll drive, the start date and a review date.

2 The rules

A note that local graduated licensing laws come first. Then the passenger rule, school-night and weekend curfew times, conditions such as highways, bad weather or dark where a parent must be present, phone rules, and an 'I will always' list of six rules the teen must all tick, including seat belts and never driving after alcohol or drugs.

3 Costs & consequences

What the teen pays for; ticking insurance asks for their monthly share. Car care duties, a required grid setting the consequence for being late, phone use, passenger breaches, tickets and alcohol or drugs, rewards for keeping the agreement, and the safe ride promise.

4 Signatures

The teen signs, then the parent gives their name, email and phone, agrees to the parent's promise and signs.
