What is the dance studio registration form template?

Dance studios register a wide mix of people — toddlers in their first ballet class, teenagers on the competition team, adults trying tap for the first time. This template handles all of them with one link. It asks 'Is the dancer under 18?' early on, and the answer changes what follows: adults give their own email and mobile, while younger dancers get a full parent or guardian page.

That guardian page asks for the parent or guardian's name and relationship, mobile and email, a second contact, who else may collect the dancer after class, and whether the parent will wait at the studio. Everyone then chooses their dance styles from ballet to ballroom, an overall level, the days and times that suit and any classes they've spotted on your timetable. Dancers with experience can name their previous studio and any exams or grades they've passed, and anyone choosing pointe is asked whether a teacher has already cleared them for it.

If the dancer is taking part in the end-of-year show, the last page collects the measurements you need to order costumes — height, chest, waist and girth — plus usual size and shoe size. A three-way photo choice, your studio terms and a first-aid consent close the form. Each signed registration is saved as a PDF in your FileIt vault.

The dance studio registration form, page by page

1 Dancer

Dancer's name and date of birth and a required 'Is the dancer under 18?'. Adult dancers give their own email and mobile; under-18s are asked for their school and year. Home address is optional.

2 Parent or guardian

Shown only for dancers under 18: the parent or guardian's name, relationship, mobile and email (all required), an optional second contact, who else may collect the dancer, and whether the parent will wait during class.

3 Classes

Required dance styles (at least one) and overall level — dancers with experience name their previous studio and any exams passed, and choosing pointe asks about teacher clearance. Required days and times that suit, specific timetable classes, interest in a performance or competition team, a required 'Will the dancer take part in the end-of-year show?' and a box for injuries, conditions or allergies.

4 Costumes & consents

For show dancers: height (required), chest, waist, girth and usual clothing size. Shoe size for everyone. Then your studio terms placeholder, a required photo and video choice, required agreement to the terms and to first aid, and the signature — the parent or guardian's when the dancer is under 18.

Make the template yours

  • Replace the terms placeholder with your fees, uniform, costume charges and withdrawal rules
  • Trim the style list to the classes you actually teach
  • Change the measurements to inches if that's how your costume supplier works
  • Rename the level options to match your studio's grades or class names
  • Add a uniform order question if dancers buy leotards or shoes through you
  • File registrations in a vault folder per term so last year's forms stay separate

Tips for a better dance studio registration form

  • Send the form well before costume ordering deadlines so measurements arrive in time
  • Use the Responses export to group dancers by style and level when building the timetable
  • Check the photo choice before posting show photos online
  • Ask families to update the form if collection arrangements change mid-term
  • Re-measure younger dancers each year — children grow faster than costume orders

Every response becomes a PDF in your vault

Each registration is filed as a signed PDF in your vault and you can get an email for each one. The family sees your confirmation and can receive a copy.

In the Responses table, search a dancer's name to check who may collect them, or export to CSV to send measurements to your costume supplier.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.

Dance studio registration form: frequently asked questions

Is the dance studio registration template free?

Yes. It's part of the Forms app on every FileIt account, including the free plan.

Can adults register themselves?

Yes. Answering no to 'Is the dancer under 18?' skips the guardian page and asks for the dancer's own contact details.

Do I have to collect costume sizes from everyone?

No. The measurements only appear for dancers taking part in the end-of-year show, and you can edit or remove them.

Can parents register more than one child?

Yes — they fill in the form once per dancer, using the same link.

How is the photo consent recorded?

Parents pick one of three choices, and the answer is printed on the signed PDF in your vault.

Do families need a FileIt account?

No. They fill in and sign the form from a link.
