What is the gymnastics class registration form template?
Gymnastics coaches need two kinds of information before a child's first session: what the gymnast can already do, and what could go wrong on the floor. This registration template asks parents for both, and adds the extra details competitive programs depend on only when they apply.
The first page covers the parent's contact details, the gymnast's name, date of birth and optional gender, and the program — preschool, recreational, tumbling and trampoline, ninja, competitive team or a try-out. Choosing the competitive team asks for the current level, the federation membership number, which meets the gymnast can attend this season and any weekends they can't compete. Every family ticks the skills the child can already do on their own, from a forward roll to a back handspring, says whether they've trained elsewhere and picks the days that suit.
The second page is medical and emergency information: a conditions checklist that asks for details and what coaches should do, medication to bring to the gym, injuries in the last year, anything else that helps coaches support the child, an emergency contact and who may collect them. Consents and the parent's signature finish it, and the PDF is filed in your FileIt vault.

Tips for a better gymnastics class registration form
- Use the skills checklist as a guide only and confirm placement in the first session
- Check federation numbers are current before entering any meets
- Share medical notes with every coach who works with the gymnast
- Ask families to tell you about new injuries, especially any concussion, before the next session
- Export meet availability to CSV when you plan travel to away meets
Every response becomes a PDF in your vault
The signed registration is saved as a PDF in your FileIt vault and you can be emailed when it arrives. The parent sees your confirmation and can receive a copy.
Use the Responses table to filter by program, or search a gymnast's name to read their medical notes before a session.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
Gymnastics class registration form: frequently asked questions
Is the gymnastics registration template free?
Yes. It's one of the starter templates in the FileIt Forms app, available on the free plan.
What extra questions do competitive gymnasts get?
Choosing the competitive team asks for the current level, federation membership number, which meets they can attend and weekends they can't compete.
Does the skills checklist place my child in a class?
No. It gives the coaches a starting point; they confirm the right group at the first session.
Can I register siblings on one form?
Each gymnast needs their own form so their medical notes and consents stay separate. Parents can use the same link again.
Is the consent wording legally checked?
No. FileIt doesn't give legal advice. Replace the placeholder and adjust the consents with wording that suits your club and country.
Do parents need a FileIt account?
No. They fill in and sign from a link.
