What is the cycling club membership form template?

A cycling club membership form has to work for very different riders: the commuter who wants a Sunday social ride, the racer who needs the club name on their licence, the family joining together and the junior whose parent signs. This template asks one question — 'Membership type' — and shows each of them only what applies.

Every rider gives their name, date of birth, mobile, email and address. Racing members are asked for their racing licence number, issuing federation, category, licence expiry date and the events they race. Family memberships list the other family members joining; juniors add a parent or guardian's name and mobile. Then everyone ticks the riding they do — road, gravel, mountain bike, cyclocross, commuting or e-bike — and picks a group ride pace, from social rides under 22 km/h to fast chain-gang style sessions, and says whether they've ridden in a pace line before.

The second page covers safety: an emergency contact, optional medical information for ride leaders, a jersey size and cut, volunteer interest and how they heard about you. Your club rules go in a placeholder, and members agree to them, to riding a roadworthy bike with a helmet and, optionally, to photos. Each signed membership is saved as a PDF in your FileIt vault.

The cycling club membership form, page by page

1 Rider & membership

Rider's name, date of birth, mobile and email (required) and address. A required membership type: racing members add a required licence number, issuing federation and category, plus licence expiry and events raced; family memberships list the other members; juniors add a parent or guardian's name and mobile. Required riding types, group ride pace and 'Have you ridden in a group or pace line before?', then any previous club.

2 Safety & club rules

A required emergency contact name and phone, relationship and optional medical information for ride leaders. Jersey size and cut, volunteer interest and how they heard about the club. Your club rules placeholder, required consents to the rules and to riding a roadworthy bike with a helmet, an optional photo consent and the signature.

Make the template yours

  • Replace the rules placeholder with your club rules, ride calls and fee details
  • Change the pace bands to the groups your club actually runs, in km/h or mph
  • Edit the racing categories to match your federation's system
  • Remove the jersey questions if you don't sell club kit
  • Add a fee summary and payment preference if you collect fees by transfer
  • Set a close date for renewal season and re-open it when you're ready

Tips for a better cycling club membership form

  • Send ride leaders the emergency contacts for their group before each season
  • Check licence expiry dates before entering racing members into events
  • Start new riders in a slower group than they choose — it's easier to move up
  • Use the volunteer answers to build a ride leader rota
  • Ask members to update their emergency contact whenever it changes

Every response becomes a PDF in your vault

Each membership is saved as a signed PDF in your FileIt vault, and the membership secretary can be emailed for every new rider. The rider sees your confirmation and can receive a copy.

Filter the Responses table by membership type to list racing members, or export to CSV to send jersey sizes to your kit supplier.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
