What is the hiking trip registration form template?

A trip leader's biggest worry is the hiker who said they were fit enough and wasn't, or the one who turns up in trainers for a mountain route. This registration template asks the questions that help a leader plan the pace, the gear check and the emergency plan before the group sets off.

The first page covers the hiker's name, date of birth and contact details, which hike or trek they're joining and the start date. They describe their fitness in plain terms — from 'comfortable walking 2–3 hours on easy paths' to 'hike 6+ hours on rough terrain, day after day' — their hiking experience, the longest or toughest hike they've done, and skills such as scrambling, river crossings, map navigation or hiking in snow. Choosing a multi-day trip adds overnight questions: sleeping arrangements (own tent, sharing with someone named, paired with another hiker or renting) and dietary needs for group meals.

The second page is a gear checklist, anything they need to borrow, a health checklist with details for the leader, medication carried and an emergency contact. Your route and risk information goes in a placeholder; hikers agree to follow the leader's decisions and confirm they've been honest about their fitness, then sign. Each registration is filed as a PDF in your FileIt vault.

Multi-day trips add a required sleeping arrangement — sharing with someone asks who — required dietary needs for group meals and a box for other food notes or allergies.

A required gear checklist and anything to borrow or rent. A required health checklist; anything other than 'None of these' asks for details for the trip leader. Medication carried, a required emergency contact name and phone, and relationship. Your trip and risk placeholder, required consents to the risks and leader's decisions and to honest answers, an optional photo consent and the signature.

  • Compare the fitness answer with the route before confirming each place
  • Carry a printed or offline list of emergency contacts and health details on the trail
  • Send the gear list again a few days before the trip
  • Pair hikers who asked to be matched by similar pace and experience
  • Ask people to tell you if their health changes between registering and the trip

Hiking trip registration form: frequently asked questions

Is the hiking trip registration template free?

Yes. It's one of the starter templates in the FileIt Forms app, available on the free plan.

What changes for multi-day trips?

Choosing a multi-day trip adds sleeping arrangements, including who you'll share a tent with, and dietary needs for group meals.

Does the form decide if someone is fit enough?

No. It records what the hiker tells you; the trip leader decides.

Can I use it for a single club hike?

Yes. Day hikes skip the overnight questions, so the form stays short.

Is the risk wording legally checked?

No. FileIt doesn't give legal advice. Replace the placeholder with wording that suits your organisation and country.

Do hikers need a FileIt account?

No. They register and sign from a link.
